营养状况对粟粒型肺结核患者病情进展影响的研究

摘　　要：目的研究营养状况对粟粒型肺结核患者病情进展的影响。方法选择2013年1月~2015年6月在医院就诊的粟粒型肺结核患者120例,根据患者是否合并急性呼吸衰竭分为ARF组32例和非ARF组88例,收集患者临床资料,包括痰菌、病程、性别、年龄、体重等,所有患者入院第二天时进行营养相关参数检查,包括胆固醇、血清白蛋白、血红蛋白以及总淋巴细胞计数,检查患者血钠水平,计算身体质量指数(BMI),采用Logistic多元回归分析粟粒型肺结核病情恶化的危险因素。结果 ARF组患者BMI、白蛋白水平、淋巴细胞计数、胆固醇水平明显低于非ARF组,差异有统计学意义(P<0.05);Logistic多元回归分析结果显示,低BMI、严重低淋巴细胞血症、低血清白蛋白、低胆固醇是导致粟粒型肺结核患者发展成为急性呼吸衰竭的独立危险因素。结论营养状况较差会促进粟粒型肺结核患者病情的恶化,临床应该在对粟粒型肺结核患者疾病的治疗同时,给予相应的营养支持。Objective To research Progress in the effect of nutritional status on the condition of miliary pulmonary tuberculosis patients. Methods 120 cases miliary pulmonary tuberculosis patients treated in the hospital were selected from 2013 January 2015 June, according to whether the patients complicated with acute respiratory failure ARF group （32 cases） and non ARF group（88 cases）. All patients were collected the clinical data, including sputum, predisposing factors, gender, age, body weight, all patients were admitted to the second day of nutrition related parameters examined, including cholesterol, serum albumin, hemoglobin and total lymphocyte count, check the serum sodium level, the calculation of body mass index （BMI）, using multivariate logistic regression analysis of miliary pulmonary tuberculosis disease progression risk factors. Results BMI, albumin, lymphocyte count, cholesterol levels of ARF patients were lower than those of the non ARF group, the difference was statistically significant （P 〈0.05）. Multivariate Logistic regression analysis results show that low BMI, severe low lymphocyte hyperlipidemia, low serum albumin and low bile cholesterol was cause miliary pulmonary tuberculosis patients develop to be an independent risk factor for acute respiratory failure. Conclusion Poor nutritional status will contribute to a worsening of miliary pulmonary tuberculosis patients, the clinical should in the treatment of miliary pulmonary tuberculosis patients at the same time, give corresponding nutritional support.
