温中止泻汤对难治型溃疡性结肠炎的疗效及患者血清IL-6、IL-10的水平影响

摘　　要：目的:研究自拟温中止泻汤对于难治型溃疡性结肠炎的临床疗效以及对患者血清白细胞介素-6(IL-6)、白细胞介素-10(IL-10)表达水平的影响。方法 :选取68例溃疡性结肠炎患者纳入本次研究,根据随机均分法进行对照组与研究组的分组,对照组给予常规治疗,研究组在对照组疗法的同时给予自拟温中止泻汤进行治疗;两组均于治疗1疗程后评估两组临床疗效并进行对比,观察两组患者治疗前、后临床主要症状的改善程度、溃疡面变化情况以及血清IL-6、IL-10表达水平的变化情况;随访6个月统计复发率。结果:对照组总有效率为70.59%,研究组总有效率为91.18%,研究临床疗效较对照组理想(P<0.05)。治疗后两组症状积分、内镜评分及血清IL-6、IL-10均有所好转,组内对比差异有统计学意义(P<0.05);治疗后研究组均优于对照组,组间对比差异有统计学意义(P<0.05)。随访6个月,对照组复发率为67.65%,研究组复发率为26.47%研究组低于对照组差异有统计学意义(P<0.05)。结论:温中止泻汤能够提高难治型溃疡性结肠炎的临床疗效,有效改善临床症状、促进溃疡面,有效降低炎性反应从而使IL-6表达水平下降、升高IL-10表达水平以拮抗炎性反应。Objective： To study the clinical effect of Wenzhong Zhixie decoction for refractory ulcerative coii-tis and the expression of serum interleukin -6 （IL-6）,interleukin-10 （IL-10） in patient. Methods： 68 cases of ulcer-ative colitis patients were included in this study, were divided into control group and scording to the method,the control group received conventional therapy,study group were treated with WenzhongZhixie decoction at the same time; after treatment to evaluate the clinical efficacy of tobserve two groups of patients before and after treatment,the degree of improvement,the mof t he ulcer and t he changes of serum IL-6,IL-10 expression; 6months follow-up the recurretotaleffective rateof thecontrol groupwas 70.59%,the total effective rateof the studygroupwas91.18%,and the clinicalefficacyof the studygroupwasbetter than that of thecontrolgroup,2 〈0.05. After treatment,symptom scores,endoscopic score and serum IL-6 and IL-10 were improved in all t wo groups,compared w i t h P〈0. 05 in t hestudygroup. After treatment,the studygroupwasbetter than thecontrolgroup,t he difference bet ween t he t wogroups was P〈0. 05. After a follow-up of 6 months,the recurrence rate of the control group was 67. 65%,the re-currence rate of the study group was 26. 47%,and that of the study group was lower than that of the control group （P〈0. 05）. Conclusion： The clinical curative effect of Wenzhong Zhixie decoction can improve refractory ulcerative colitis,improve clinical symptoms,promote ulcer surface,effectively reduce the inflammatory reaction and the ex-pression of IL-6 decreased and increased the expression of IL-10 in antagonistic anti inflammatory response.
