小青龙汤对外寒内饮型哮喘患儿肺功能及血清炎性因子的影响

摘　　要：目的:结合临床实践经验,探讨小青龙汤对于外寒内饮型哮喘患儿肺功能及血清炎性因子、血气分析的作用。方法:选取外寒内饮型哮喘患儿112例,按照随机数表法,随机分为治疗组和对照组各56例,对照组患者给予常规对症治疗,主要包括止咳平喘、解痉、扩血管、祛痰、抗感染等。治疗组患者在此基础上给予小青龙汤进行联合治疗,水煎服,1次/d,15d为1疗程。分别观察两组治疗疗效,并对两组肺功能(VC、TLC、FVC、MVV)、血清炎性因子(CRP水平)、血气(TNF-α、PaO_2、PaCO_2)等指标进行比较,并观察两组患儿治疗过程中不良反应发生情况。结果:经过治疗,对照组总有效率(87.5%)明显低于治疗组(96.43%),差异有统计学意义(P<0.05);治疗后治疗组肺功能各指标(VC、TLC、FVC、MVV)优于对照组,差异有统计学意义(P<0.05);治疗前,两组血清炎性因子和血气指标无统计学差异(P>0.05),治疗后,两组CRP、TNF-α、PaO_2、PaCO_2水平均有所改善,且对照组改善程度明显不如治疗组,差异有统计学意义(P<0.05)。两组患儿治疗过程中均未出现明显不良反应。结论:小青龙汤对于外寒内饮型哮喘患儿有显著疗效,可提高患者肺功能,减轻患儿炎性反应,改善患儿血清炎性因子水平,对于患儿血气运行也有积极的促进作用,促进血气恢复运行。Objective:Combined with clinical practice experience,the effects of Xiaoqinglong decoction on lung function and serum inflammatory factor and blood gas analysis of children with external cold drink were discussed.Methods:Type selection outside in the cold drink asthmatic children 112 cases,according to the stochastic indicator method,each 56 cases were randomly divided into observation group and control group,control group patients give conventional symptomatic treatment,mainly including cough and asthma,spasmolysis,expanding blood vessel,expectorant,anti-infection and so on.On this basis,patients in the observation group were given a combination of treatment,water decoction,1 time/d and 1 course of treatment.Respectively to observe the curative effect in both groups,and the two groups of lung function(VC,TLC,FVC,MVV),serum inflammatory factors(CRP levels),blood gas(TNF alpha,PaO_2 and PaCO_2)index to compare,and observe the adverse reactions in two groups of children with treatment process.Results:After treatment,the total effective rate(87.5%)was significantly lower than the observation group(96.43%),and the difference was statistically significant(P<0.05).After treatment,the lung function of the observation group(VC,TLC,FVC,MVV)was better than the control group,and the difference was statistically significant(P<0.05).Before the treatment,two groups of serum inflammatory factors and blood gas index no statistical difference(P> 0.05),after treatment,two groups of CRP,the level of TNF alpha,PaO_2,PaCO_2 was improved,and the control group improved significantly as observation group,the difference was statistically significant(P<0.05).There was no obvious adverse reaction in the treatment of the two groups.Conclusion:Xiaoqinglong decoction for asthmatic children have significant curative effect,can improve the patients' lung function and reduce inflammatory reaction,serum levels of inflammatory cytokines in children with blood gas operation also has a positive role in promoting,promotes the flesh to restore operati
