摘 要:局部脑氧饱和度(rSO_2)监测是一种新型无创监测脑氧平衡的方法,通过评估脑部氧的供需平衡状况和脑血流量变化情况指导临床应用。目前,rSO_2的正常阈值尚未达成共识,但其主要与脑氧供、脑氧耗及红外线吸收的相关影响因素有关。术中实时监测rSO_2并将其维持在一定范围内可以优化围术期管理,降低术后神经系统并发症发生率、缩短住院时间、改善患者预后。rSO2监测最初主要用于心脏外科及深低温停循环手术的脑保护中,随着临床应用增多,rSO_2逐渐用于神经外科、重症监护室及体外心肺复苏中,用于评估神经系统预后情况。Regional cerebral oxygen saturation( rSO2) monitoring is a new noninvasive method to monitor cerebral oxygen balance. It can guide the clinical application through the assessment of cerebral oxygen supply and demand balance and changes in cerebral blood flow. At present,the reference ranges of rSO2 has not reached a consensus,but it is mainly related to factors of cerebral oxygen supply-demand balance and infrared absorption. Intraoperative real-time monitoring of rSO2 and maintenance within a certain range can optimize perioperative management and reduce the incidence of postoperative neurological complications as well as improve the patient's prognosis. Initially rSO2 monitoring is mainly used for the brain protection in cardiac surgery and deep hypothermic circulatory arrest,while with the clinical application increase,it is gradually used in neurosurgery,intensive care,extracorporeal cardiopulmonary resuscitation,and assessing the prognosis of the nervous system.
