摘  要:目的探讨预先应用甲状腺素片对脓毒症大鼠肿瘤坏死因子-α(TNF-α)和肺组织病理的影响。方法大鼠尾静脉注射脂多糖(LPS,5mg/kg)建立脓毒症模型。36只雄性Wistar大鼠随机分为空白对照组(C组)、脓毒症组(S组)、甲状腺素预先干预组(L组),每组12只。L组预先左旋甲状腺素钠(10μg/100g)灌胃7d,S组和c组等量生理盐水灌胃7d。建模后0、3、6、12、24h采血,测血清TNF-a、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)水平,24h处死大鼠并取肺组织,测肺组织湿/干质量比(W/D),行HE染色并免疫组化。结果FT3、FT4在0h各组均无变化(P〉0.05),s组、L组3h降低,6h最低,然后逐渐上升,在3、6、12、24h等时间点较c组明显降低(P〈0.01),而在3、6、12、24h时间点L组较S组明显升高(P〈0.01)。S组和L组W/D较c组升高(P〈0.01),较S组升高更明显(P〈0.01)。病理显示,c组肺泡结构完整;S组肺泡壁塌陷,间隔增宽,炎细胞浸润明显;L组较s组减轻。血清TNF-α在S组和L组3h升高,6h达峰值,较C组升高(P〈0.01),较S组升高更明显(P〈0.01);肺组织TNF-α阳性细胞数量和染色强度s组最多、最强,L组次之,c组最少、最弱。结论预先应用甲状腺素片能降低脓毒症大鼠血清TNF-α水平,改善肺组织病理改变。Objective To investigate the effect of thyroxine pretreatment on tumor necrosis factor-ct (TNF- a) and lung pathology in septic rats. Methods LPS (5 mg/kg) was injected into the caudal vein of rats to establish the sepsis model. Thirty - six male Wistar rats were randomly divided into blank control group (group C ) , sepsis group (group S ) and thyroxine pretreatment group (group L) , with 12 rats in each group. Group L was given by gavage with levothyroxine sodium ( 10 μg/100 g) for 7 days, and equivalent physiological saline were administered in the same protocol without levothyroxine sodium for 7 days in group S and group C. Rats in each group were drew blood after modeling for 0, 3, 6, 12, 24 h. And the levels of TNF - a, free three triiodothyro ( FT3 ) and free thyroxine (FT4) were measured. Rats were sacrificed after 24 h and lung tissue was taken to measure the wet/dry weight (W/D) , and HE staining and immunohistochemistry were performed. Results The level of FT3 and FT4 was not significanlg different among each group at 0 h ( P 〉 0.01 ). However itbegan to decrease at 3 h, and decreased to a minimum at 6 h in group S and group L. The FT3, FT4 level in group L and group S was lower than that in group C ( P 〈 0.01 ), and reduced more markedly in group S (P〈0.01). The W/D in group S and group L was higher than that in group C (P〈0.01), and elevated more markedly in group S (P 〈 0.01 ). The pathological findings showed that the alveolar structure of group C was intact. However in group S, the alveolar wall collapsed with alveolar septum broadening, and the inflammatory cell infiltration was obvious. Compared with group S, the pathological changes of group L were obviously reduced. The serum TNF -α concentration started to elevate at 3 h, and reached a peak at 6 h in group S and group L.
