摘  要:目的探讨多部门联合对新生儿重症监护室(Neonatal Intensive CareUnit,NICU)抗菌药物合理使用的干预效果。方法分别抽取本院NICU病区2015年1月出院病历(干预前)及2016年1月出院病历(干预后),对实施多部门联合干预前后抗菌药物用药合理性和病原学指标进行比较。结果经多部门联合干预后,NICU病区抗菌药物使用强度由干预前的28下降为14;干预前以加酶抑制剂复合制剂和美罗培南为主,干预后基础性药物二、三代头孢用量上升明显;干预前后均以单用为主,联合用药情况变化不明显;药物选择、给药频次、用药疗程、标本采集与送检时间、所选药物感染部位敏感、致病菌、病历中抗菌药物用药分析指标合理比例均有明显提高,差异有统计学意义(P〈0.05)。干预前后排序前五位细菌变化不明显;标本送检率较干预前有所提高。结论多部门联合对NICU抗菌药物合理使用的干预是可行并且有效的。Objective To study the effect of multiseetoral joint intervention on the rational use of antibiotics in neonatal intensive care unit (NICU). Methods The patients discharged from the NICU of our hospital in January 2015 (before the intervention) and the patients discharged from the NICU of our hospital in January 2016 (after the intervention) were selected, and the rationality of antimicrobial agents and pathogen indicators before and after multisectoral joint intervention were compared. Results The intensity of antimicrobial agents in the NICU reduced from 28 before the intervention to 14 after multiseetoral joint intervention. Before the intervention, the combination of enzyme inhibitors and meropenem were the main drugs, and the dosage of the second and third generations of cephalosporins significantly increased after intervention. Before and after the intervention the single medication was main, the change of combination medication was not obvious. The rational ratio of drug selection, frequency of medication, course of medication treatment, specimen collection and inspection time, selected drugs sensitive to infection site, pathogens, and analysis of antimicrobial drug use in the medical record significantly improved (P 〈 0.05). The changes of the first five bacteria were not obvious before and after the intervention. The sampling rate after the intervention was higher than that before the intervention. Conclusion Multisectoral joint intervention on the rational use of antimicrobial agents of NICU is feasible and effective.
