摘 要:目的研究合理氧疗联合乙酰半胱氨酸对慢性阻塞性肺疾病急性加重期的疗效。方法选取本院2015年4月至2016年4月接收的120例慢性阻塞性肺疾病急性加重期患者作为研究对象,随机将所有研究对象分为对照组和实验组,各60例。对照组给予乙酰半胱氨酸,实验组在给予乙酰半胱氨酸的基础上给予合理氧疗。比较两组患者在治疗前后血清抑制羟自由基能力、谷胱甘肽硫转移酶(GSH—ST)活力、抗超氧阴离子能力、PaCO2、PaO2、FEV1/FVC(%)和FEV1%pred水平。结果经过治疗后,两组GSH—ST活力、抗超氧阴离子能力和血清抑制羟自由基能力测定值优于治疗前,并且实验组患者改善程度优于对照组患者,两组比较差异有统计学意义(P〈0.05);两组PaO2和PaC02测定值优于治疗前,并且实验组患者改善程度优于对照组患者,差异有统计学意义(P〈0.05);两组FEV,%pred和FEV1/FVC测定值显著优于治疗前,差异有统计学意义(P〈0.05);并且实验组患者改善程度优于对照组患者,差异有统计学意义(P〈0.05)。结论合理氧疗联合乙酰半胱氨酸能够提高肺泡内氧分压,增加氧弥散能力,增加组织供氧,降低肺动脉压力,减轻呼吸困难程度,有利于提高治疗效果。Objective To study the effect of N-acetylcysteine combined with oxygen therapy in the treatment of AECOPD patients. Methods 120 cases of AECOPD patients in our hospital between April 2015 and April 2016 were selected as the research object and were randomly divided into control group and experimental group, 60 cases in each group. The control group was given N-acetylcysteine, and the experimental group was given N-acetyleysteine combined with oxygen therapy. The levels of serum inhibition ability to hydroxyl free radical, glutathione s transferase (GSH-ST) vitality, the ability of resistance to the ultra oxygen anion, PaCO2, PaO2, FEV1/FVC (%), and FEVl%pred before and after treatment of the two groups were observed. Results After treatment, the GSH-ST vitality, the ability of resistance to the ultra oxygen anion, and serum inhibition ability to hydroxyl free radical of the two groups were significantly better than those before treatment, the improvement degree of the experimental group was better than that of the control group, with statistically significant difference between the two groups (P〈0.05). After treatment, the levels of PaO2 and PaCO2 of the two groups were significantly better than those before treatment, the improvement degree of the experimental group was better than that of the control group, with statistically significant difference between the two groups (P〈0.05). After treatment, the levels of FEV~/FVC (%) and FEV1%pred of the two groups were significantly better than those before treatment, the improvement degree of the experimental group was better than that of the control group, with statistically significant difference between the two groups (P〈0.05). Conclusion N-acetylcysteine combined with oxygen therapy can improve the alveolar oxygen partial pressure, increase oxygen dispersion ability and oxygen supply, reduce pulmonary artery pressure and the degree of difficulty in breathing, and improve the treatment effect.
