丹参川芎嗪注射液对老年顽固性心力衰竭合并低血压患者心肾功能的改善作用

摘　　要：目的探讨丹参川芎嗪注射液对老年顽固性心力衰竭合并低血压患者心肾功能的改善作用。方法选取老年顽固性心力衰竭合并低血压患者共100例,按随机数字表法分为观察组和对照组各50例。在常规治疗的基础上,对照组给予多巴胺联合多巴酚丁胺治疗,观察组给予丹参川芎嗪注射液治疗。比较两组患者治疗前后SBP、DBP、心率、血浆肾素活性、血管紧张素Ⅱ、醛固酮、血清肌酐(SCr)、左心室射血分数(LVEF)、每搏输出量(SV)、N末端前体脑钠肽(NT-proBNP)及估算肾小球滤过率(e GFR)。结果与治疗前比较,治疗后两组SBP、DBP、SV、LVEF、eGFR均增高,血浆肾素活性、血管紧张素Ⅱ、Nt-proBNP、SCr水平均降低(P<0.05),观察组心率及醛固酮水平均降低,对照组醛固酮水平升高(P<0.05)。治疗后观察组心率、血浆肾素活性、血管紧张素Ⅱ、醛固酮、Nt-proBNP、SCr均低于对照组,SV、LVEF、eGFR均高于对照组(P<0.05)。结论丹参川芎嗪注射液可有效改善老年顽固性心力衰竭合并低血压患者的心肾功能。Objective To investigate the effect of Danshen Chuanxiongqin injection on the heart and kidney function improvement in elderly patients with refractory heart failure and hypotension. Methods A total of 100 elderly patients with refractory heart failure and hypotension were divided into observation group and control group according to the random number table method, with 50 cases in each group. Besides the conventional therapy, dopamine combined with dobutamine and Danshen Chuanxiongqin injection were given to the control group and the observation group, respectively. SBP, DBP, heart rate, plasma renin activity, angiotensin H , aldosterone, serum creatinine （ SCr ）, left ventricular ejection fraction （ LVEF）, stroke volume （ SV ）, N -terminal pro-brain natriuretic peptide （ NT-proBNP ） and estimated glomerular filtration rate（eGFR） before and after treatment were compared between the two groups. Results After treatment, SBP, DBP, SV, LVEF and eGFR increased while plasma renin activity and the levels of angiotensin II , NT-proBNP and SCr diseased in both groups compared to the indices above before treatment（ P 〈 0.05 ）, both heart rate and aldosterone level decreased in the observation group, and the level of aldosterone increased in the control group （ P 〈 O. 05 ）. After treatment, heart rate, plasma renin activity and the levels of angiotensin H , aldosterone, NT-proBNP and SCr were lower but SV, LVEF and eGFR were higher in the observation group compared to the indices above in the control group （ P 〈 0. 05 ）. Conclusion Danshen Chuanxiongqin injection can effectively improve the heart and kidney function of elderly patients with refractory heart failure and hypotension.
