摘 要:目的观察低能量体外冲击波(ESW)对膝骨关节炎大鼠软骨细胞凋亡的影响。方法将30只裸大鼠按随机数字表法分为空白组(n=10)、模型组(n=10)、低能量ESW组(n=10);模型组与低能量ESW组均建立KOA模型,空白组不予以建模;建模6周后,拆除石膏,低能量ESW组予以低能量(0.1 m J/mm2)ESW治疗,冲击1000次。4周后,处死大鼠,行组织形态大体观察及染色观察,并采用TENUL方法对软骨细胞凋亡情况进行检测。结果干预4周末,空白组软骨外观显光亮蓝白色,无裂痕、缺损,软骨结构、潮线显示清楚;模型组膝关节呈现典型的KOA病理改变;而低能量ESW组软骨触摸有软化,少许脱落,潮线模糊。低能量ESW组Moran评分为(5.93±0.71)分,明显高于模型组的(4.32±0.76)分(P<0.05);低能量ESW组Mankin评分、细胞凋亡率分别为(8.03±1.07)分、(12.12±1.59)%,均明显低于模型组的(9.78±1.76)分、(14.25±2.23)%,差异有统计学意义(P<0.05)。结论低能量ESW可有效抑制KOA大鼠软骨细胞凋亡,对KOA治疗有重要参考价值。Objective To observe the effects of low energy extracorporeal shock wave(ESW) on chondrocyte apoptosis in rats with knee ostcoarthritis. Methods The 30 nude rats were divided into the blank group(n=10),the model group (n=lO) and the low energy ESW group(n=lO) by random number table method. The model group and the low energy ESW group were given the establishment of the KOA model, and the blank group was not modeled. After 6 weeks of modeling, the piaster was removed and the low energy ESW group was treated with low-energy(0.1 mJ/mm2) ESW with 1000 shocks. After 4 weeks,the rats were killed and the morphological observation and staining observation were carried out. The chondrocytes apoptosis was detected by TENUL. Results At the end of 4 week of intervention,there were bright blue cartilages without cracks and defects ,and the clear cartilage structure and tidal line in the blank group. The typical KOA pathological changes were shown in the knee joint in the model group while the cartilage touch was softened with a little fell off,and the tidal line was blurred in the low energy ESW group. The score of Moran in the low energy ESW group was significantly higher than that in the model group[(5.93±0.71) points vs (4.32±0.76) points] (P〈 0.05). The Mankin score and chondrocyte apoptosis rate were significantly lower than those in the model group [(8.03± 1.07) points vs (9.78±1.76) points; (12.12±1.59)% vs (14.25±2.23)%] (P〈0.05). Conclusion Low energy ESW can effectively inhibit the chondrocytes apoptosis in KOA rats, and has important reference value for the treatment of KOA.
