依那普利联合螺内酯治疗老年慢性心力衰竭的临床疗效观察

摘　　要：目的探讨依那普利联合螺内酯治疗老年慢性心力衰竭(CHF)的临床效果。方法选择2015年6月~2017年6月在我院住院治疗的78例CHF患者,根据治疗方法不同随机分为观察组和对照组,每组39例。两组患者均予休息、限制食盐摄入及利尿剂、洋地黄、β受体阻滞剂等常规治疗。观察组同时加用依那普利10 mg,每天2次,螺内酯20 mg/d,每天1次,疗程3个月,比较两组治疗后的疗效及心功能。结果观察组治疗后的临床总有效率显著高于对照组,组间比较差异具有显著性(P<0.05),观察组治疗后的LVEF、心率明显高于对照组,组间比较差异具有显著性(P<0.05),但两组患者治疗前的LVEF、心率比较,差异无显著性(P>0.05)。结论对老年慢性心力衰竭患者应用依那普利联合螺内酯治疗,可以显著提高临床疗效,改善患者的心功能,提高患者的生存质量。Objective To investigate the clinical efficacy of enalapril combined with spironolactone in treatment of elderly patients with chronic heart failure. Methods 78 CHF patients hospitalized in our hospital from June 2015 to June 2017 were selected. According to the different treatment methods, the patients were randomly divided into observation group and control group, with 39 patients in each group. Both groups were given conventional treatment including rest and limitation of salt intake, diuretics, digitalis and beta blockers. The observation group was also given enalapril 10 rag, 2 times a day, spironolactone 20 mg/d, once a day. And the treatment course was 3 months. The effi- cacy and cardiac function after treatment were compared. Results The total effective rate of the observation group after treatment was significantly higher than that of control group, and the difference between two groups was significant（P〈 0.05）. The LVEF and heart rate of the observation group was significantly higher than that of control group, and the difference was significant（P〈0.05）. But there were no significant differences in LVEF and heart rate between the two groups before treatment（P〉0.05）. Conclusion The application of enalapril combined with spironolactone in treatment of elderly patients with chronic heart failure can significantly improve the clinical efficacy, improve the patients＇ cardiac function and improve the quality of life of patients.
