昆山出入境人员二类疫苗接种收费标准的合理性调查

摘　　要：目的探讨出入境人员二类疫苗接种收费标准的合理性与改进方向。方法以昆山出入境检验检疫局2016年度二类疫苗麻疹-风疹-腮腺炎(MMR)、流脑、水痘疫苗接种为例,分析疫苗采购、冷链保存、疫苗损耗、注射耗材、不良反应防范及人力等成本,结合现行定价下的预期收入探讨现行收费标准的合理性;结合疫苗管理条例修订和药品零差率改革探讨定价改进方向。结果 2016年昆山地区MMR、流脑、水痘疫苗的收费标准分别为每支77、67、148元,接种人次为175、61、28人次,总收入为21 706元;3种疫苗流通过程中,疫苗采购、注射耗材、冷链保存、人力成本、不良反应防范成本为21 480.0、258.8、197.9、7 764.0、117.1元,总成本为29 817.8元,全年亏损8 111.8元。结论按现行的二类疫苗接种定价标准收费,出入境人员二类疫苗接种收不抵支;通过控制采购数量、适当提高预防接种服务性收费、适当增加财政补贴等举措,可避免资源浪费、实现收支平衡。Objective To analyze the cost and income of the type Ⅱvaccine inoculation in Kunshan Entry-Exit Inspection and Quarantine Bureau,and to provide scientific basis for discussing the reasonableness and improvement direction of the type II vaccination charge standard. Methods Measles rubella mumps （MMR） vaccine,Meningitis vaccine,Varicella vaccine immanized at Kunshan Entry-Exit Inspection and Quarantine Bureau were selected as examples, analyzing the vaccine procurement, logistics, injection supplies, adverse reactions prevention and manpower costs,combined with the policy and the current price of expected income. The policy rationality and the direction of pricing improvement had been discussed in combination with the revision of the vaccine management regulations and the reform. Results In 2016,the charging standard of MMR,Meningitis vaccine,Varicella vaccine was 77,67 and 148 yuan,respeetively,and the total amount of inoculation was 175,61 and 28,with total income of 21 706 yuan. The cost of circulation process,vaccine procurement,injecting supplies,logistics cost,manpower cost,adverse reactions prevention was 21 480,258.8,197.9,7 764,117.1 yuan,the total cost was 29 817.8 yuan. The annual cost was 8 111.8 yuan. Conclusion The income of type Ⅱ vaccines vaccination was less than the cost according to the policy. It is necessary to reduce the purchase number,raise the immunization price and subsidy,to avoid waste,increase income.
