摘 要:目的评价关节镜下肘关节松解术治疗军事训练伤致肘关节僵硬的临床疗效。方法回顾性分析作者科室2013-10/2016-08月因军事训练伤致肘关节僵硬需行松解术的军人28例,所有病例均接受关节镜下肘关节松解术治疗,术后辅以手法松解,配合镇痛及康复理疗锻炼。对比手术前、后的活动度,应用Mayo评分系统进行疗效评价。结果术后随访6~9(8.4±1.3)个月,术前肘关节平均最大屈曲角度为(86.5±5.2)°、伸直角度为(34.7±4.7)°、旋前角度为(76.4±5.5)°、旋后角度为(67.1±5.4)°,与术后的最大屈曲角度(125.4±5.6)°、伸直角度(5.1±1.2)°、旋前角度(80.6±4.3)°、旋后角度(80.4±4.4)°比较差异均有统计学意义(P<0.05)。肘关节Mayo评分,手术前平均为(60.9±4.4)分,手术后平均为(87.1±5.1)分,手术前后比较差异有统计学意义(P<0.05)。结论肘关节镜松解术作为微创手术,具有创伤小、恢复快、术野清晰及松解彻底等优点,术后肘关节活动度明显改善,在军事训练伤致肘关节僵硬的治疗中具有显著疗效。Objective To evaluate the clinical effects of arthroscopic arthrolysis for elbow stiffness caused by mili- tary training injuries. Methods Twenty-eight elbow stiffness patients caused by military training injury were performed arthroseopic arthrolysis in authors" department from October 2013 to August 2016, and retrospectively analyzed. All pa- tients were treated manipulation arthrolysis, analgesia and healing treatment after surgery. The activities of pre- and post- operation were evaluated by Mayo classification and compared. Results After 6-9 (8. 4± l. 3) months of post-operative follow-up, preoperative average maximum flexion angles of elbow joint, straight angle, pronation angle, rotational angle were (86. 5 ±5. 2), (34. 7 ± 4. 7), (76. 4 ± 5.5), (67. 1± 5. 4) °, respectively, and postoperative above angles, in turn, were (125. 4 ± 5. 6), (5. 1± 1.2), (80. 6± 4. 3), (80. 4 ± 4. 4)°. There was significantly different between pre- and post-opera- tion (P〈0. 05). The preoperative average elbow joint assessment were (60. 9 ± 4. 4) scores and postoperative were (87. 1 ±5.1) scores by Mayo classification (P〈0.05). Conclusion Arthroseopic arthrolysis for elbow stiffness has advantages of clear operative area, minimally invasive and rapid postoperative recovery. It has an effective improvement of elbow joint range of motion and remarkable effects on elbow stiffness caused by military training.
