摘  要:目的:观察Er:YAG激光及传统翻瓣刮治术对种植体尖周病损的治疗效果。方法:将符合纳入条件的患者随机分为两组:实验组根尖部翻瓣后采用Er:YAG去除感染组织及对种植体根尖部进行清洁处理后行GBR;对照组行传统的翻瓣刮治术,刮治器刮净根尖肉芽组织,碳化钨钢钻切除感染的种植体根尖部后行GBR,术后1周、2周、1月、3月、6月、12月观察种植体动度,黏膜健康状况,拍摄X线根尖片观察种植体根尖骨的变化。结果:收集符合纳入标准的病例8例,传统手术治疗4例,有1例完全恢复正常,1例处于静止状态,2例治疗失败取出后重新种植;Er:YAG激光治疗4例,有2例完全恢复正常,2例处于静止状态。结论:同传统翻瓣刮治术相比,Er:YAG激光治疗种植体尖周病损在保存种植体方面有较好的效果。但其远期效果有待进一步的观察。Objective: To observe the effect of Er:YAG laser and traditional flap scaling on the treatment of periapical implant lesion(IPL). Methods: The patients who met the inclusion criteria were randomly divided into two groups: after the root tips flap scaling, the infections were removed used Er:YAG and treated with GBR in the experimental group, traditional flap scaling was used, granulation tissues were removed, the infected implant apex was dissected to use carbide bur and treated with GBR in the contol group, then the implant mobility, mucosal health, bone of implant apex were observed 1w, 2w, 1m, 3m,6, 12m after operation. Results: 8 cases were collected in accordance with the inclusion criteria, 4 cases were treated with traditional operation, 1 case was fully recovered, 1 case was still and 2 cases were replanted after treatment failure. Er:YAG laser treatment 4 cases, 2 cases completely recovered, 2 cases were at rest. Conclusion: Compared with traditional flap scaling, Er:YAG laser therapy has a better effect on the preservation of implants. But its long-term effects need a further observation.
