摘 要:目的了解臂丛神经根性撕脱伤合并膈神经损伤的概率,为膈神经移位修复术提供临床依据。方法对2011年1月至2014年1月在我院治疗的150例臂丛神经根性撕脱伤患者进行临床随访,通过术前和术中电生理检测来评价膈神经功能。结果150例臂丛神经根胜撕脱伤患者中合并膈神经损伤患者48例(32.0%),其中膈神经部分损伤患者25例(16.7%),完全损伤患者23例(15.3%);而不合并膈神经损伤患者102例(68.0%)。损伤类型、年龄、性别、受伤性质及受伤侧不同的臂丛神经根性撕脱伤患者在膈神经损伤程度之间差异无统计学意义(P〉0.05)。结论臂丛神经根性撕脱伤患者合并膈神经损伤的概率不高,大部分患者膈神经功能正常,故可作为动力神经来修复损伤的臂丛神经。Objective To understand the probability of brachial plexus avulsion injuries combined with phrenic nerve lesion and provide the clinical basis for the repair of brachial plexus using phrenic nerve transfer. Methods From January 2011 to January 2014, a retrospective study was carried out on 150 patients with unilateral brachial plexus avulsion injuries. Their phrenic nerve function was evaluated through pre- and intra-operative electromyography. Results The phrenic nerve was normal in 102 cases (68.0%), partially injured in 25 cases (16.7%) and completely injured in 23 cases (15.3%). There was no significant difference in the degree of phrenic nerve lesion between different brachial plexus avulsion injuries, including injury type and property, patients' age and sex, and injured side (P〉0.05). Conclusion The patients with brachial plexus root avulsion do not have a high probability of phrenic nerve lesion. Most of the patients have normal phrenic nerve function, so it can be used as the dynamic donor nerve to repair the injured brachial plexus.
