摘　　要：目的:评价医院-社区一体化冠心病管理模式的干预效果。方法:收治冠心病患者90例,平分两组。对照组予以常规管理,观察组予以医院-社区一体化冠心病管理。比较两组干预效果、生活质量。结果:与对照组干预效果比较,观察组的较高,优势突出(P<0.05);与对照组生活质量比较,观察组的较高,优势突出(P<0.05)。结论:医院-社区一体化冠心病管理模式可有效改善冠心病患者的生活方式与习惯,提高生活质量,值得借鉴。Objective：To evaluate the intervention effect of hospital-community integrated coronary heart disease management.Methods：90 patients with coronary heart disease were selected.They were divided into the two groups on average.The control group was given regular management.The observation group received hospital-community integrated coronary heart disease management.The effect of intervention and the quality of life were compared.Results：Compared with the control group,the observation group had higher advantages and prominent advantages（P0.05）.Compared with the control group,the quality of life in the observation group was higher and the advantage was prominent（P0.05）.Conclusion：The hospital-community integrated coronary heart disease management model can effectively improve the lifestyle and habits of patients with coronary heart disease,and improve the quality of life.It is worth reference.
