阿托伐他汀联合复方丹参滴丸治疗DR伴高血脂的临床研究

摘　　要：目的探讨阿托伐他汀钙片联合复方丹参滴丸治疗糖尿病视网膜病变(DR)合并高血脂的临床疗效。方法随机将2015年3月~2017年2月本院收治的118例单纯型DR合并高血脂的患者分为A组(对照组59例)和B组(观察组59例),两组均给予常规基础降糖治疗,此外,A组给予复方丹参滴丸治疗,10丸/次,3次/d,连续使用两个疗程(8 w);B组在A组治疗基础上,加服阿托伐他汀钙片,10 mg/次,1次/d,连续使用两个疗程(8 w)。观察比较两组治疗后的视力、眼底情况、血脂改善情况、血液流变学指标及不良反应情况。结果治疗两个疗程后,B组的显效率及总有效率显著优于A组(P<0.05);B组甘油三酯(TG)、血清总胆固醇(TC)、低密度脂蛋白(LDL-C)3项指标均明显低于A组(P<0.05),高密度脂蛋白(HDL-C)指标明显高于A组(P<0.05);B组血液流变学指标改善情况也显著优于A组(P<0.05)。两组治疗期间均出现1例便秘不良反应,未出现其他严重不良反应。结论单纯型DR合并高血脂患者采用阿托伐他汀钙片联合复方丹参滴丸的治疗方案,不仅可以有效提高其临床疗效,而且还可有效改善血脂及血液流变学指标,建议临床推广使用。Objec t ive To explore the clinical efficacy of atorvastatin calcium tablets combined with compound Danshen dropping pills in the treatment of diabetic-retinopathy （DR） complicated with hyperlipidemia. MeAods A total of 118 patients who had been suffering from simple DR complicated with hyperlipidemia and admitted to our hospital from March 2015 to February 2017 were randomly divided into group A and group B （n =59, respectively）. Routine and basic hypoglycemic treatment was performed on both groups, compound Danshen dropping pills were additionally given to group A for two continuous courses （8 w）, 10 pills/time and three times per day, and atorvastatin calcium tablets were given to group B for two continuous courses （8 w） on the basis of the treatment togroup A, 10 mg/time and one time per day. Observation and comparisons were made between the two groups with respect to the vision, fundus condition, blood lipid improvement, hemorheological indexes and adverse reactions. Results After two courses of treatment, the markedly effective rate and total effective rate in group B were higher than those in group A （P 〈 0.05）; the triglyceride （TG）, the serum total cholesterol （TC） and the low density lipoprotein （LDL-C） in group B were significantly lower than those in group A （P 〈 0.05）, while the high density lipoprotein （HDL-C） was higher than that in group A （P 〈 0.05）; the hemorheological indexes in group B were improved more significantly than those in group A （P 〈 0.05）. One constipation case in both groups was found during the treatment, but no other sever adverse reactions occurred. Con clusion The treatment therapy of atorvastatin calcium tablets combined with compound Danshen dropping pills in patients with simple DR complicated with hyperlipidemia can not only effectively improve the clinical efficacy but also lead to effective improvement of blood lipid and hemorheological indexes; and hence it is recommended to promote the use
