电针联合艾灸治疗创伤性应激障碍的临床研究

摘　　要：目的探讨针灸在创伤性应激障碍(PTSD)疾病中的临床应用价值。方法将骨折后继发PTSD患者116例按随机数字表法分为治疗组64例与对照组52例,治疗组采用电针联合艾灸,同时配合口服帕罗西汀进行治疗;对照组单纯采用口服帕罗西汀治疗。结果治疗组痊愈率为85.94%,明显高于对照组之72.73%(P<0.05);两组患者CAPS临床疗效比较,治疗组总有效率为93.75%,明显高于对照组之76.92%(P<0.05);治疗组和对照组患者在治疗4、8、12周末创伤后应激障碍自评量表(PTSD)、汉密尔顿焦虑量表(HAMA)、事件影响量表(IES-R)、汉密尔顿抑郁量表(HAMD)积分均较同组治疗前显著改善(P<0.05);治疗组改善显著(P<0.05)。治疗组和对照组不良反应发生率分别为6.25%、21.15%,差异具有统计学意义(P<0.05)。结论电针联合艾灸对PTSD治疗效果优于西药帕罗西汀,可以作为PTSD联合治疗方法之一。Objective： To explore the clinical value of acul;uncture on PTSD. Methods： 116 patients with traumatic mental disorders secondary to fracture were randomly divided into two groups： the treatment group and the control group. The treatment group was treated with electroacupuncture combined with moxibustion and oral paroxetine,while the control group was treated with paroxetine. Results： The recovery rate of the treatment group was 85.94%,which was significantly higher than that of the control group （72.73%）, and the difference was statis- tically significant （P〈 0.05）. compared with CAPS clinical effects of the two groups,the total effective rate of the treatment group was 93.75% ,which of the control group was 76.92% ,and the total effective rate of the treatment group was better than that of the control group （P〈 0.05）. The scores of PTSD-SS,HAMA and HAMD in the treatment group and the control group 4,8 and 12 weeks after treatment were significantly different than those be- fore the treatment （P〈 0.05） and the treatment group were significantly different from those of the control group （P 〈 0.05）. The incidence of adverse reactions in the treatment group and the control group was 6.25% and 21.15%, respectively,with significant difference （P〈 0.05）. Conclusion： The effect of electroacupuncture combined with Moxibustion on PTSD is better than that of Western medicine paroxetine. It can be used as one of the combined treatment methods of PTSD.
