仙方活命饮合五味消毒饮加减在骨折内固定术后应用

摘　　要：目的观察仙方活命饮合五味消毒饮加减在骨折内固定术后应用。方法选择我院行骨折内固定术患者82例,依据随机数字表法随机分为观察组41例与对照组41例。对照组于患者术后采用常规处理,观察组在对照组基础上结合仙方活命饮合五味消毒饮加减治疗。比较两组患者术后切口感染情况、术后1d和术后4周血清超敏C-反应蛋白(hs-CRP)和中性粒细胞计数变化、疼痛视觉模拟(VAS)评分变化,及术后1周和4周骨折线和内外骨痂评分变化。结果观察组术后切口感染率明显低于对照组(P<0.05);两组术后4周血清hs-CRP和中性粒细胞计数均明显降低(P<0.05);观察组术后4周血清hs-CRP和中性粒细胞计数均低于对照组(P<0.05);两组VAS评分术后4周明显降低(P<0.05);观察组VAS评分术后4周明显低于对照组(P<0.05);观察组骨折线和内外骨痂评分术后1周、术后4周均明显高于对照组(P<0.05);观察组临床治疗总有效率明显高于对照组(P<0.05)。结论仙方活命饮合五味消毒饮加减应用于骨折内固定术后效果显著,具有重要研究意义。Objective To observe the effect of Xianfang Huoming decoction combined with modified Wuwei Xiaodu decoction in treatment of fracture fixation. Methods 82 cases of patients with internal fixation of fracture were randomly divided into observation group and control group, 41 cases in each group. Control group received conventional treatment, observation group received Xianfang Huoming decoction combined with modified Wuwei Xiaodu decoction on the basis of control group. Infection of incision after operation were compared between the two groups. The changes of serum high sensitivity C- reactive protein （hs-CRP） and neutrophil count, pain visual analogue scale （VAS） score and the changes of fracture line and internal and external callus score at 1 and 4 weeks after operation were compared after operation. Results The infection rate of observation group was significantly lower than that of control group （P〈0.05）; after 4 weeks of operation, serum hs-CRP and neutrophil counts were significantly decreased in both groups （P〈0.05）; serum hs-CRP and neutrophil count in observation group was lower than that in control group （P〈0.05）; VAS scores of both groups significantly decreased （P〈0.05）, that of obsesvation group was significantly lower than control group （P〈0.05）; the fracture line and internal and external callus score in observation group were significantly higher than those in control group at the 1st week and the 4th week after the operation （P〈0.05）. The total effective rate of clinical treatment in observation group was significantly higher than that in control group （P〈0.05）. Conclusion Xianfang Huoming decoction combined with modified Wuwei Xiaodu decoction is effective in treatment of fracture internal fixation with important research significance.
