摘  要:目的探讨冠心病患者血浆色素上皮衍生因子(pigment epithelium-derived factor,PEDF)含量与冠脉造影狭窄程度的关系。方法纳入2016年10月-2017年3月就诊于本院心血管内科的冠心病(coronary artery disease,CAD)患者190例。依据冠心病患者造影狭窄程度(Gensini评分)将冠心病患者分为轻度病变组(Gensini评分≤26分,平均年龄61.51±9.70岁,男性41例,女性26例)、中度病变组(26<Gensini评分≤45分,平均年龄61.76±9.61岁,男性42例,女性20例)和重度病变组(Gensini评分>45分,平均年龄63.92±9.13岁,男性47例,女性14例)。测定3组患者血浆PEDF水平,进而分析PEDF水平与冠状动脉狭窄严重程度的关系。结果轻度病变组、中度病变组、重度病变组患者血浆PEDF水平分别为(6.42±0.59)μg/ml、(5.79±0.75)μg/ml、(5.35±0.73)μg/ml,差异有统计学意义(P=0.000)。随着冠状动脉Gensini评分增高,患者血浆PEDF水平进一步降低,两者呈负相关(β=-0.552,P<0.01)。结论冠心病患者血浆PEDF水平与冠状动脉狭窄程度呈负相关,PEDF有作为冠心病狭窄程度预测因子的潜力。Objective To investigate the relationship between the content of pigment epithelium-derived factor (PEDF) and degree of stenosis by coronary angiography in patients with stable coronary artery disease (CAD). Methods One hundred and ninety consecutive stable CAD patients admitted to our hospital from October 2016 to March 2017 who received coronary angiography were enrolled in our study. According to the degree of stenosis (Gensini score) of coronary angiography, patients were divided into three groups: mild group (Gensini score ≤ 26, 41 males and 26 females, average age of 61.51 ± 9.70 years), moderate group (26 〈 Gensini score ≤ 45, 42 males and 20 females, average age of 61.76 ± 9.61 years), and severe group (Gensini score 〉 45, 47 males and 13 females, average age of 63.92 ± 9.13 years). PEDF levels were measured in patients of three groups and the relationship between PEDF level and degree of stenosis was analyzed. Results The plasma PEDF levels in mild, moderate and severe group were (6.42 ± 0.59) μ g/ml, (5.79 ± 0.75) Ix g/ml and (5.35 ± 0.73) μg/ml, respectively, which showed significant differences between three groups (P=0.000). With the increase of the coronary artery Gensini score, the plasma PEDF levels of the patients decreased accordingly. Multiple regression analysis revealed that PEDF level was negatively correlated with degree of stenosis (β =-0.552, P 〈 0.01). Conclusion Our study indicates that plasma PEDF level can serve as a potential predicator for degree of coronary stenosis in CAD patients.
