尿道下裂术后两种不同尿道支架引流方式的疗效比较

摘　　要：目的：探讨不同材质的尿道支架管在尿道下裂修复术后的应用效果。方法：选取2013年6月-2016年6月在我院行尿道下裂修复术的患儿82例,随机分为实验组和对照组,每组41例。对照组术后应用多孔硅胶尿道支架管,试验组术后应用镍钛记忆合金尿道支架管,观察两组患者的拔管时间、住院时间、术区感染率、术后尿道狭窄和尿瘘发生率、初次排尿痛发生率、膀胱刺激症发生率。结果：两组患者的拔管时间、住院时间、膀胱刺激症发生率无明显差异（P〉0.05）,不具有统计学意义;实验组患者术区感染率、术后尿道狭窄和尿瘘发生率、初次排尿痛发生率均明显低于对照组（P〈0.05）。结论：镍钛记忆合金尿道支架具有引流和支架的双重作用,有利于减少尿道下裂修复术后尿瘘、尿道狭窄的发生率,在减轻患者疼痛,提高手术成功率方面具有明显优势。Objective： To investigate the effect of urethral stent with different materials on the repair of hypospadias. Methods： Totally 82 children with hypospadias in our hospital from June 2013 to June 2016 were randomly divided into experimental group and control group, with 41 cases in each group. The control group was treated with porous silicone urethral stent, and the experi- mental group was treated with NiTi memory alloy urethral stent. The extubation time, hospital stay, operative area infection rate, postoperative urethral stricture and urinary fistula were ob- served as well as the initial incidence of urination pain and the incidence of bladder irritation. Results： There was no significant difference in extubation time, hospitalization time and incidence of bladder irritation between the two groups （P〉0.05）. The infection rate, postoperative urethral stricture and incidence of urinary fistula, and the incidence of urination in the operation group were significantly lower than those in the control group （P〈0. 05）. Conclusion： NiTi memory alloy urethral stent has dual functions of drainage and stent. It is helpful to reduce the incidence of urinary fistula and urethral stricture after hypospadias repair, and it has obvious ad- vantages in relieving pain and improving the success rate of operation.
