摘 要:目的探讨重度子痫前期孕妇血清胱抑素C(Cys—C)、补体C1q(C1q)的水平变化及临床意义。方法选择2016年1月至12月重度子痫前期孕妇85例为观察组,另选择同期孕周和年龄与重度子痫前期组相匹配的血压正常、无产科合并症及并发症的健康孕妇95例为对照组,常规检测两组孕妇入院时血清肌酐(SCr)、尿素氮(BUN)、血清胱抑素C(Cys—C)、补体C1q(C1q)的水平,比较两组孕妇上述指标的差异。结果观察组孕妇八院时血清肌酐、尿素氮分别为(52.15±11.39)μmol/L、(4.72±1.38)mmol/L,对照组孕妇入院时血清肌酐、尿素氮分别为(4118±1045)μmol/L、(3.76±3.44)mmol/L,两组比较差异无统计学意义(P〉0.05);观察组孕妇入院时血清胱抑素C、补体C1q水平分别为(1.36±0.31)mg/L、(176.32±30.81)mg/L,对照组分别(111±0.21)mg/L、(161.74±16.64)mg/L,两组比较差异有统计学意义(P〈0.05)。结论重度子痫前期孕妇常伴随肾功能异常改变,血清胱抑素C、补体C1q水平的异常变化先于血清肌酐、尿素氮,可以作为一个肾功能早期损伤指标加以重视,如发现异常改变应密切随访、及时复查。Objective To evaluate and analyze the clinical significance of serum cystatin C ( Cys-C ) and complement C 1 q ( C 1 q ) in pregnant women with severe preeclampsia. Method 85 cases of severe preectampsia pregnant women from January 2016 to December were selected as the observation group, 95 healthy pregnant women with normal blood pressure, no obstetric complications and complications were selected for the same period as a control group. The levels of serum creatinine ( SCr ) , urea nitrogen ( BUN ) , serum cystatin C ( Cys-C ) and complement Clq ( Clq ) were measured in two groups of pregnant women. The differences of the above indexes in two groups of pregnant women were compared. Result in the observation group, the serum creatinine and urea nitrogen were ( 52.15 -+ 11.39 ) mol/L and ( 4.72± 1.38 ) mmol/L, respectively, when pregnant women were admitted to hospital. In the control group, the serum creatinine and urea nitrogen were ( 41.18 ± 10,45 ) mol/L and ( 3.76 ± 3.44 ) mmol/L,respectively. There was no significant difference between the two groups (P〉0.05) . The levels of serum cystatin C and complement Clq in the observation group were ( 1.36±0.31 ) mg/L and ( 176.32±30.81 ) mg/L,respectively. The control group was ( 1.11±0.21 ) mg/L and ( 161.74 ± 16.64 ) mg/L,respectively. The difference between the two groups was statistically significant ( P〈0.05 ) . Conclusion Pregnant women with severe preeclampsia often accompany abnormal changes in renal function. The abnormal changes of serum creatinine and urea nitrogen were preceded by serum cystatin C and complement Clq, which can be regarded as an early index of renal function injury. If abnormal changes are found, close follow-up and timely review should be made.
