IgG、IgM表达在不稳定心绞痛诊断中的价值及临床意义分析

摘　　要：目的探讨血IgG、IgM表达对不稳定心绞痛的诊断价值,及其与肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、组织型纤溶酶原激活物(t PA)、组织型纤溶酶原激活物抑制物-1(PAI-1)水平的相关性。方法选取2016年2月~2017年4月于山东省莱芜市人民医院心内科接受治疗的不稳定心绞痛患者80例为研究对象,同时选取同期在我院接受体检的80名健康成年人作为对照。观察两组IgG、IgM阳性表达情况,比较两组血清TNF-α、CRP、t PA、PAI-1和心功能指标的差异,分析不稳定心绞痛患者IgG、IgM表达与TNF-α、CRP、t PA、PAI-1和心功能指标的相关性。结果不稳定心绞痛组患者的IgG阳性表达率高于对照组,两组的IgM阳性表达率无差别;不稳定心绞痛组患者血清TNF-α、CRP、PAI-1水平高于对照组,两组的t PA水平无明显差别;不稳定心绞痛组患者的左心房内径(LAD)、右心房内径(RAD)和脑钠肽(BNP)水平高于对照组,LVEF水平低于对照组;不稳定心绞痛组患者的IgG阳性表达率与TNF-α、CRP、PAI-1、LAD、RAD和BNP水平正相关,与LVEF水平负相关;而IgM阳性表达率与TNF-α、CRP、t PA、PAI-1和心功能指标等无明显相关关系。结论不稳定心绞痛患者的IgG阳性表达较高,且与TNF-α、CRP、PAI-1水平密切相关。Objective To investigate the value of IgG and IgM in the diagnosis of unstable angina pectoris and its correlation with the levels of TNF-α, CRP, tPA and PAI-1. Methods The patients with unstable angina pectoris treated from February 2016 to April 2017 were selected as the subjects, and the healthy adults who underwent physical examination were selected as the control group. To observe the expression of two groups of IgG and IgM, the difference between the two groups of serum TNF-α, CRP, tPA, PAI-1 and heart function index, correlation analysis in patients with unstable angina IgG, the expression of IgM and TNF-α, CRP, tPA, alpha PAI-1 and cardiac function index. Results The positive rate of IgG in the unstable angina pectoris group was higher than that in the control group, and there is no difference of the positive rate of IgM between the two groups. The serum levels of TNF-α, CRP and PAI-1 in the patients with unstable angina pectoris were higher than those in the control group, and there was no significant difference in the levels of tPA between the two groups. The levels of LAD, RAD and BNP in the patients with unstable angina pectoris were higher than those in the control group, and the level of LVEF was lower than that of the control group. Unstable angina in patients with IgG positive expression rate of TNF-α and CRP, PAI-1, alpha, LAD, RAD and BNP correlated inversely related to LVEF levels, while the positive expression rate of IgM and TNF-α, CRP, tPA, alpha PAI-1 and heart function index has no significant correlation. Conclusion The positive expression of IgG in patients with unstable angina pectoris is higher, and is closely related to the levels of TNF-α, CRP and PAI-1.
