摘  要:目的:探讨甲状腺功能亢进患者血清Ⅰ型胶原氨基端前肽(N-terminal propeptide of typeⅠprocollagen,PINP)水平与骨密度的相关性。方法:选择2014年9月至2016年3月收治的甲状腺功能亢进患者90例为研究对象,应用放射免疫法检测血清PINP水平,根据治疗前后血清PINP水平变化将90例患者分为控制有效组和控制较差组,检测两组患者血清Ⅰ型胶原羧基端肽(carboxyterminal propeptide of typeⅠprocollagen,PICP)、N端中段骨钙素(amino-terminal and midcourse osteocalcin,N-MID OT)、β-胶原特殊序列(β-Cross Laps)、甲状旁腺素(parathyroid hormone,PTH)、25-羟基维生素D[25-(OH)Vit D]、血清磷、钙以及碱性磷酸酶(alkaline phosphatase,ALP)水平。结果:PINP控制有效组患者血清PICP、β-Cross Laps、N-MID OT、25-(OH)Vit D及PTH水平均高于控制较差组(P<0.05),而控制有效组患者的血钙、血磷、ALP水平低于控制较差组(P<0.05);患者骨密度与血清PINP水平呈负相关(r=-0.436,P=0.007)。结论:甲亢患者骨密度与血清PINP水平呈负相关,甲亢患者应注意预防骨质疏松。Objective:To investigate the correlation between bone mineral density(BMD) in patients with hyperthyroidism and serum N-terminal propeptide of type 1 procollagen(PINP).Methods:90 patients with hyperthyroidism hospitalized from September 2014 to September 2015 were chosen,with PINP levels determined by radioimmunoassay method.The 90 patients were divided into the effective control group and the poor control group according to serum PINP levels before and after the treatment,with carboxyterminal propeptide of typeⅠprocollagen(PICP) amino-terminal and midcourse osteocalcin(N-MID OT),beta collagen specific sequences(β-Cross Laps,parathyroid hormone(PTH),25 hydroxy vitamin D[25-(OH) Vit D],serum phosphorus,serum calcium and levels of serum alkaline phosphatase(ALP) determined in the two groups.Results:TPINP,β-Cross Laps,N-MID OT,25-(OH) Vit D,PTH levels in the effective control group was significantly higher than those of the poor control group,the difference being statistically significant(P〈0.05).The serum calcium,phosphorus and ALP levels in the effective control group were significantly lower than those in the poor control group,the difference being statistically significant(P〈0.05).BMD in patients with hyperthyroidism and serum PINP levels showed a negative correlation(r=-0.436,P=0.007).Conclusion:There is a negative correlation between BMD in patients with hyperthyroidism and serum PINP levels,so patients with hyperthyroidism should pay attention to the prevention of osteoporosis.
