曼月乐与安宫黄体酮治疗围绝经期异常子宫出血的临床疗效对比

摘　　要：目的:比较围绝经期异常子宫出血患者采用曼月乐(左炔诺孕酮宫内缓释系统,LNG-IUS)与安宫黄体酮治疗的临床疗效。方法:本院2014年8月—2016年8月收治的围绝经期异常子宫出血患者88例,随机分为观察组和对照组各44例,观察组采用LNG-IUS治疗,对照组采用安宫黄体酮治疗。比较两组治疗前及治疗3、6、12个月时的月经量、经期时间、子宫内膜厚度、血红蛋白(Hb)、红细胞比容(Hct),统计临床疗效和不良反应发生情况。结果:与治疗前比较,两组治疗3、6、12个月的月经量、经期时间和子宫内膜厚度均显著改善,Hb和Hct均升高(均P<0.05),两组间比较有显著差异(P<0.05);观察组治疗总有效率(97.7%)高于对照组(84.1%)(P<0.05);两组均未见严重不良反应。结论:对于围绝经期异常子宫出血患者,给予曼月乐治疗较安宫黄体酮治疗效果好,值得临床推广。Objective：To investigate the clinical effect of levonorgestrel releasing intrauterine system（LNG-IUS）compared to medroxyprogesterone for treating perimenopausal abnormal uterine bleeding.Methods：From August 2014 to August 2016,88 women with perimenopausal abnormal uterine bleeding in Baoji maternal and child health care Hospital of Shaanxi province were randomly divided into observation group and control group.Women in observation group were treated by LNG-IUS inserted,and women in control group were treated by medroxyprogesterone acetate.The menstrual volume,menstruation time,endometrial thickness,hemoglobin（Hb）and hematocrit（Hct）of women were compared between the two groups before treatment and in 3,6,and 12 months after treatment.And rates of adverse reactions of women in both groups were also analyzed.Results：The menstruation volume,menstrual period,endometrial thickness of women in both groups in 3,6,12 months after treatment were significant decreased than those of women before treatment（P〈 0.05）,and value of Hb and Hct had significant increased than those of women before treatment（P〈0.05）.The total effective rate of women in observation group was 97.73%,which was significant higher than that of women（84.09%）in control group（P 〈 0.05）.There was no any serious adverse reaction in both groups.Conclusion：LNG-IUS sued for treating abnormal uterine bleeding of perimenopause women has better clinical effect than that of medroxyprogesterone acetate,which is worthy of promotion.
