摘  要:目的:探讨代谢综合征患者和肥胖者血清YKL-40水平的变化及其与胰岛素抵抗的关系。方法:选取于佳木斯大学附属第一医院内分泌科就诊的代谢综合征患者42例(Met S组),另选体检健康BMI≥25.0kg/m^2者23例作为超重和(或)肥胖组(Obe组),体检健康且BMI≤25.0kg/m2者30例作为正常对照组(NC组)。采用ELISA检测血清YKL-40水平。结果:Met S组血清YKL-40水平显著高于NC组,差异有统计学意义(P<0.01)。Obe组和NC组间YKL-40差异无统计学差异(P>0.05)。YKL-40与HOMA-IR、FPG、Hb A1c、FIns、TG、TC呈正相关(均P<0.05)。多元逐步回归分析结果显示,FPG和FIns是血清YKL-40水平的独立相关因素。结论:YKL-40与胰岛素抵抗呈正相关,YKL-40与代谢综合征的微炎症状态相关。Objective:To investigate the changes of serum YKL-40 levels in patients with metabolic syndrome and obesity and its relationship with insulin resistance.Methods:42 patients with metabolic syndrome( Met S group) attending the department of Endocrinology,the First Affiliated Hospital of Jiamusi University were enrolled in this study.23 healthy persons with BMI≥25.0 kg/m2 were selected as overweight and/or obese group( Obe group),30 healthy person with BMI ≤25.0 kg/m^2 as normal control group( NC group).Serum YKL-40 levels was measured by ELISA.Results:Serum YKL-40 levels in Met S group were significantly higher than those in NC group,and the difference was statistically significant( P < 0.01).There was no significant difference in YKL-40 between Obe group and NC group( P > 0.05).YKL-40 was positively correlated with HOMA-IR,FPG,HbA1c,FIns,TG and TC( all P < 0.05).Multiple linear stepwise regression analysis showed that FPG and FIns were independent influencing factors of serum YKL-40 levels.Conclusion:YKL-40 is positively correlated with insulin resistance,and YKL-40 is associated with micro-inflammatory status of metabolic syndrome.
