三维成形环治疗风湿性心脏瓣膜病继发性三尖瓣关闭不全的疗效观察

摘　　要：目的探讨三维成形环治疗风湿性心脏瓣膜病继发性三尖瓣关闭不全的疗效。方法选取在我院就诊的风湿性心脏瓣膜病继发性三尖瓣关闭不全患者72例,按随机数字表法分为两组,各36例。对照组行Kay氏成形术或DE Vega瓣环成形术治疗,观察组予三维成形环三尖瓣成形术,比较两组右心房内径(RAD)、右心室前后径(RVD)、三尖瓣反流面积(TVR)、心排出量(CO)水平及并发症情况。结果术后6个月,观察组TVR明显低于对照组,CO水平较高,差异有统计学意义(P<0.05);两组并发症发生率及RVD、RAD水平比较,差异无统计学意义(P>0.05)。结论三维成形环可缩小风湿性心脏瓣膜病继发三尖瓣闭合不全患者三尖瓣反流面积,增加心排出量。Objective To evaluate the efficacy of three-dimensional ring in the treatment of secondary tricuspid insufficiency in rheumatic valvular disease. Methods A total of 72 patients with secondary tricuspid insufficiency in rheumatic valvular disease in our hospital were selected and randomly divided into two groups,with 36 cases in each group.The control group was treated with Kay's plasty or DE Vega valvuloplasty,and the observation group was treated with three tricuspid valvuloplasty.The levels and complications of right atrial diameter(RAD),right ventricular anterior diameter(RV),tricuspid regurgitation area(TVRV) and cardiac output(CO) were compared between the two groups. Results At 6 months after operation,the level of TVR in the observation group was significantly lower than that in the control group(P < 0.05).There was no significant difference in the incidence of complications and the level of RVDG RAD between the two groups(P > 0.05). Conclusion Three-dimensional ring can reduce the area of tricuspid regurgitation and increase cardiac output in patients with rheumatic valvular disease secondary tricuspid insufficiency.
