摘 要:目的观察评价视频眼震电图仪诊断良性阵发性位置性眩晕患者的眼震特征及定位的临床效果。方法选择2014年6月~2016年12月期间我院耳鼻咽喉科收治的良性阵发性位置性眩晕(BPPV)的患者60例,在变位试验检查中采取的视频眼震电图仪进行分析眼震情况与定位。并把同期60例患者设置为对照组进行裸眼观察BPPV患者在变位试验检查时眼震情况。结果治疗1周后,观察组的总有效率为95.00%与对照组93.33%比较,差异无统计学意义(χ~2=0.1517,P>0.05);治疗3个月后,观察组的总有效率为100.00%高于对照组98.33%有统计学意义(χ~2=4.0084,P<0.05);60例患者中共诊断后半规管BPPV的患者则是42例占70.00%,VNG及裸眼辅助下典型眼震检出率依次是92.86%(39/42)及66.67%(28/42),两者间有统计学意义(χ~2=7.7647,P<0.05)。前半规管BPPV患者10例占16.67%(10/60),VNG及裸眼辅助下典型眼震的检出率分别为100.00%(10/10)及70.00%(7/10),两者比较差异有统计学意义(χ~2=3.5294,P<0.05)。共诊断水平半规管BPPV患者5例占8.33%(5/60),VNG及裸眼辅助下典型眼震的检出率分别为8例及6例。结论 VNG准确反映了并观察BPPV患者的眼震发生的状况,可以显著地提高眼震的检出率及准确诊断,值得临床推广应用。Objective To observe the clinical effect of video nystagmograph electrogram in the diagnosis of nystagmus characteristics and localization of benign paroxysmal positional vertigo. Methods Sixty patients with benign paroxysmal paroxysmal dizziness(BPPV) were enrolled in the department of otorhinolaryngology of our hospital from June 2014 to December 2016. The analysis of nystagmus characteristics and localization by the video nystagmograph electrogram used in the displacement test examination. And 60 patients were set up as the control group for naked eye observation of BPPV patients in the displacement test examination of nystagmus. Results After 1 week of treatment, the total effective rate was 95.00% in the observation group and 93.33% in the control group(χ~2=0.1517, P > 0.05). After treatment for 3 months, the total effective rate of the observation group was 100.00%, and higher than that of the control group(98.33%), χ~2= 4.0084, P < 0.05. In 60 patients, 40 cases(70.00%) with semicircular canal BPPV were diagnosed, VNG and naked eye were used to detect the typical nystagmus, and the rates were 92.86%(39/42) and 66.67%(28/42), respectively, there was significant difference between the two groups(χ~2=7.7647, P < 0.05). Ten cases of BPPV patients in the anterior semicircular canal accounted for 16.67%(10/60).The detection rate of typical nystagmus in VNG and naked eye was 100.00%(10/10) and 70.00%(7/10), respectively, and there was a significant difference(χ~2=3.5294, P < 0.05). The detection rate of typical nystagmus was 5.33%(5/60), VNG and naked eye were detected in 8 cases and 6 cases. Conclusion VNG accurately reflects and observes the occurrence of nystagmus in BPPV patients, which can significantly improve the detection rate and accurate diagnosis of nystagmus, and is worthy of clinical application.
