摘 要:目的探讨彩色多普勒超声联合胃超声造影剂对原发性十二指肠肿瘤(PTD)的诊断价值。方法选择自2012年1月至2017年7月于沈阳军区总医院就诊的51例PTD患者为研究对象。所有患者均行彩色多普勒超声联合胃超声造影剂及胃镜检查,观察肿瘤发生位置、大小、形态及累及范围等,比较两种检查方法的检出率。结果 51例PTD患者中,鳞癌8例,腺癌31例,腺瘤5例,神经内分泌瘤2例,淋巴瘤2例,间质瘤3例,彩色多普勒超声联合胃超声造影剂检出率为86.3%(44/51),显著高于胃镜检出率60.7%(31/51),差异有统计学意义(P<0.05)。5例十二指肠球部肿瘤,胃镜全部检出,检出率为100.0%(5/5),彩色多普勒超声联合胃超声造影剂漏诊1例,检出率为80.0%(4/5),漏诊肿瘤位于十二指肠球后壁,两种检查方法比较,差异无统计学意义(P>0.05);27例大乳头周围及降部肿瘤,胃镜检出21例,检出率为77.8%,彩色多普勒超声联合胃超声造影剂检出23例,检出率为85.2%,两种检查方法比较,差异无统计学意义(P>0.05);19例十二指肠水平部及升部肿瘤,胃镜检出5例,检出率为26.3%,彩色多普勒超声联合胃超声造影剂检出17例,检出率为89.4%,两种检查方法比较,差异有统计学意义(P<0.05)。结论彩色多普勒超声联合胃超声造影剂检查简便易行,患者痛苦小,具有广泛应用价值,可作为一种无创性检查方法应用于十二指肠肿瘤的筛查诊断。Objective To investigate the ultrasonic performance of primary tumors of the duodenum( PTD) by color ultrasonography combined with gastric contrast agent,and to evaluate the ultrasound diagnostic value of PTD. Methods A retrospective study was performed on 51 cases of patients with PTD who were admitted from January 2012 to July 2017. Patients were detected by color Doppler ultrasounography combined with gastric ultrasound contrast agents,the location,size,morphology and tumor range of tumor were observed,and the results of two methods were compared. Results Among 51 cases of patients with PTD,there were 8 cases of squamous carcinoma,31 cases of adenocarcinoma,5 cases of adenoma,2 cases of neuroendocrine tumor,2 cases of lymphoma,3 cases of interstitial tumor,color doppler ultrasound combined contrast detection rate was 86. 3%( 44/51),significantly higher than that of gastroscope detection rate 60. 7%( 31/51),and the difference was statistically significant( P 〈 0. 05). In 5 cases of duodenal bulb tumor,the gastroscope all detected,1 case was misdiagnosed by color doppler ultrasound combined with contrast agent,and the tumor was located in the posterior wall of duodenal bulb; there were 27 cases of large nipple and descending tumor,21 cases were detected by gastroscope,and 23 cases were detected by color doppler ultrasound combined with contrast agent. In the 19 cases of duodenal level and ascending tumor,5 cases were detected in gastroscope and 17 cases were detected by color doppler ultrasound combined with contrast agent. Conclusion The detection rate of PTD is higher by color Doppler ultrasounography combined with gastric ultrasound contrast agents,with the advantages such as simple operation,small pain of patients and non-invasively,which is worth of clinical promotion.
