摘  要:目的探讨超声造影后粗针穿刺活检技术在颈部肿大淋巴结鉴别诊断中的应用,为临床提供有价值的参考依据。方法选取自2015年5月至2016年5月在沈阳军区总医院就诊的73例颈部淋巴结肿大患者为研究对象,将所有患者随机分为A组(n=37)和B组(n=36)。A组患者行超声引导下粗针活检穿刺,B组患者行超声造影后粗针活检穿刺,比较两组患者的取材成功率、诊断准确率及并发症发生率。结果 B组患者取材成功率为100.0%(36/36),诊断准确率为100.0%(36/36),均高于A组的取材成功率89.2%(33/37)及诊断准确率89.2%(33/37),差异有统计学意义(P<0.05);B组患者的并发症发生率为0,低于A组的8.1%(3/37),差异有统计学意义(P<0.05)。结论超声造影后粗针穿刺颈部肿大淋巴结活检能提高取材成功率及诊断准确率,减少穿刺过程中的并发症。Objective To investigate the application of the coarse core biopsy after the contrast-enhanced ultrasonography in differential diagnosis of swollen neck lymph nodes,in order to offer the valuable reference to clinical diagnosis. Methods A retrospective study was performed on 73 cases of patients with swollen neck lymph nodes who were admitted from May 2015 to May 2016. Patients were randomly divided into Group A( n = 37) and Group B( n = 36). Patients in Group A received coarse core biopsy under the guidance of ultrasonography,the others in Group B received coarse core biopsy after the contrast-enhanced ultrasonography. The success rate,diagnostic accuracy and complication rate of the two groups were compared. Results Biopsy success rate and diagnostic accuracy rate in Group B were higher than those of Group A,100. 0%( 36/36) versus 89. 2%( 33/37) and 100. 0%( 36/36) versus 89. 2%( 33/37),( P 〈 0. 05); the incidence of complications in Group B was lower than that in Group A( 8. 1%,3/37),( P 〈 0. 05).Conclusion The coarse core biopsy after the contrast-enhanced ultrasonography in diagnosis of neck lymph nodes can improve biopsy success rate,diagnostic accuracy rate and reduce incidence of complications.
