摘 要:目的探讨超声引导下经皮穿刺双管引流在治疗巨大胰腺假性囊肿中的应用价值。方法选择沈阳军区总医院自2012年6月至2017年6月收治的65例巨大胰腺假性囊肿患者为研究对象,根据采用的引流方案将患者分为单管引流组(n=38)与双管引流组(n=27)。比较两组患者腹胀缓解时间、淀粉酶恢复正常时间、抗生素使用时间及引流时间的差异。结果 65例患者全部成功置管,穿刺成功率为100%,均未出现较严重并发症。2例单管引流患者发生引流管堵塞,但超声显示囊肿仍存在,故进行第2次超声引导下穿刺置管。双管引流组患者的腹胀缓解时间、淀粉酶恢复正常时间、抗生素使用时间及引流时间均短于单管引流组,差异有统计学意义(P<0.05)。结论超声引导下经皮穿刺双管引流具有微创、安全有效、并发症少等优点,治疗巨大胰腺假性囊肿效果显著。Objective To investigate the application of percutaneous puncture and double-tube drainage under ultrasound guidance in the treatment for giant pancreatic pseudocyst. Methods A retrospective study was performed on 65 cases of patients with giant pancreatic pseudocyst who were admitted from June 2012 to June 2017. According to different drainage methos,patients were divided into the single-tube drainage group( n = 38) and double-tube drainage group( n = 27). The differences in duration of abdominal distension,normal time of amylase recovery,time of antibiotic use and drainage time were compared between the two groups. Results All the 65 patients had successful catheters,and the success rate of puncture was 100%. No serious complications occurred. Two patients with single-tube drainage were blocked by drainage tube,while the ultrasound showed that the cyst still existed,the second time of ultrasound guided puncture drainage was performed. The duration of abdominal distension,normal time of amylase recovery,time of antibiotic use and drainage time in the double-tube group were all shorter than those in the single-tube drainage group,and the difference was statistically significant( P 〈 0. 05). Conclusion Double-tube drainage of ultrasound-guided percutaneous puncture has the advantages of minimally invasive,safe and effective with less complications,the effect of treating giant pancreatic pseudocyst is remarkable.
