摘　　要：目的探讨季节变化对冠状动脉血管成像(CTA)筛查阳性率的影响。方法选取自2016年9月至2017年7月在沈阳军区总医院进行冠状动脉CTA检查的患者10 726例为研究对象。根据患者行CTA检查的时间,将患者分为春季组(n=3 524)、夏季组(n=2 657)、秋季组(n=2 115)及冬季组(n=2 430)共4组。观察并比较各组患者的CTA阳性率。结果夏季组患者中冠状动脉CTA完全正常的比例较高,为38.8%(1 031/2 657),而春季和冬季冠状动脉CTA发现冠状动脉粥样硬化症或冠状动脉粥样硬化性心脏病的比例总和高于夏季及秋季。女性患者的CTA完全正常的比例明显高于男性患者,尤其是夏季。结论对于女性患者,如果有胸痛症状,建议首先进行功能学检查评估是否有心肌缺血的存在,尤其是夏季的女性胸痛患者,不建议首选CTA进行冠状动脉粥样硬化性心脏病的筛查。Objective To investigate the effects of season changes on positive rate of CT angiography（ CTA） examination.Methods A retrospective study was performed on 10 726 cases of patients who were performed CTA examination from September2016 to July 2017. According to the time of CTA examination,patients were divided into the spring group（ n = 3 524）,summer group（ n = 2 657）,autumn group（ n = 2 115） and winter group（ n = 2 430）. The positive rate of CTA in all groups were observed and compared. Results The perfectly normal proportion of CTA in the summer group was higher,accounting for 38. 8%（ 1 031/2 657）. The combined ratio of coronary atherosclerosis or coronary atherosclerotic heart disease was higher in spring and winter groups than in those in the summer and autumn groups. The perfectly normal proportion of CTA in female patients was significantly higher than that of male patients,especially in summer. Conclusion For female patients,if she has chest pain,it is recommended that the first function studies evaluated the presence of myocardial ischemia,especially female patients with chest pain in summer; it is not recommended for CTA preferred coronary atherosclerotic heart disease screening.
