通痹方中药热奄包对颈型颈椎病家兔颈椎曲度及椎间盘组织形态学的影响

摘　　要：目的:研究通痹方中药热奄包干预对颈型颈椎病模型家兔颈椎形态和组织学的影响。方法:选用6月龄健康雄性新西兰家兔24只,随机均分为空白对照组、模型组及外敷组,模型组和外敷组家兔采用自制保湿冰袋使兔低头屈曲位方法制备颈型颈椎病模型,空白对照组家兔不做任何干预,造模完成后外敷组给予通痹方中药热奄包热敷(1次/d,20~25 min/次),5 d为1疗程,共3个疗程;各组家兔于造模前、造模后及干预结束时拍摄颈椎X线片并计算积分,干预结束后次日取颈椎间盘组织观察形态学变化。结果:与空白对照组比较,模型组颈椎X线片显示颈椎曲度存在显著病理性改变,X线积分明显升高(P<0.01);HE染色见颈椎间盘纤维环、髓核等发生退变;与模型组比较,外敷组干预治疗后颈椎X线片显示颈椎曲度显著改善,但椎体前后缘骨赘未见明显变化;X线积分显著降低(P<0.01);HE染色见颈椎间盘纤维环、髓核等改善明显。结论:通痹方中药热奄包可有效改善颈椎曲度,缓解颈椎间盘的退行性改变。Objective： To observe the effects of Tongbi herbal Reyanbao on the morphology of inter- vertebral disc tissue and cervical curvature in rabbit cervical spondylosis. Methods ：24 healthy male New Zealand Rabbits were randomly divided into control group, model group, herbal Reranbao external apphcation group. The model of cervical spondylosis was established by using self-made moisturizing ice bag and lowering rabbit head to make flexion position in model group and external application group. Rabbits in control group did not accept any intervention. After modeling, external application group was given Tongbi herbal Reyanbao ,5 d for the 1 course of treatment, a total of 3 courses. All the rabbits in each group were taken cervical X-ray films before and after modeling and at the end of the intervention, and the integration was calculated. Cervical disc tissue was taken at the next day after the intervention to observe the morphological changes. Results：Compared with control group, the cervical X-ray films of the model group showed a significant pathological change in the cervical curvature, and the X-ray score increased obviously（P 〈0.01 ）. HE staining showed the degeneration in annulus fibro- sus and the nucleus pulposus of the cervical intervertebral disc. Compared with the model group, the cervical vertebra X-ray in external application group after the intervention showed significant improve- ment in cervical curvature, but there was no significant change in osteophyma of tile anterior and poste- rior margin of the vertebral body. X-ray score was significantly reduced （ P 〈0. 01 ） ; HE staining showed that annulus fibrosus and nucleus pulposus of the cervical intervertebral disc improved signifi- cantly. Conclusion： External application of Tongbi herbal Reyanbao can effectively improve cervical curvature, relieve cervical intervertebral disc degeneration.
