三种红细胞沉降率测定方法的临床应用

摘　　要：目的:评价意大利Vital、ALIFAX Roller 20自动血沉仪与魏氏法检测红细胞沉降率(ESR)结果的一致性。方法:采用意大利Vital、ALIFAX Roller 20自动血沉仪与魏氏法评价其检测ESR的一致性;随机选取30例标本,分析两种自动血沉仪与魏氏法检测ESR的相关性;选取正常和异常血沉标本2例,用自动血沉仪重复测定10次评价其精密度;选取30例标本放置4.5 h,每30 min用动血沉仪检测血沉,观察血液标本放置时间对血沉结果的影响。结果:两种自动血沉仪与魏氏法检测ESR结果一致性极强(K≥0.81)、相关性好(r≥0.971)、精密度良好(正常组CV%≤7.70%、异常组CV%≤4.94%);Vital自动血沉仪检测标本放置时间超过3 h,对测定结果有影响,检测结果比较差异有统计学意义(P<0.05);ALIFAX Roller 20自动血沉仪检测标本放置时间超过1.5 h,对测定结果有影响,检测结果比较差异有统计学意义(P<0.05)。结论:Vital自动血沉仪和ALIFAX Roller 20自动血沉仪与魏氏法检测ESR有极强的一致性,可缩短报告时限。Objective： To evaluate the consistency of erythrocyte sedimentation rate （ESR） detected by Italian Vital, ALIFAX Roller 20 automatic ESR analyzer and referenced Westergren method. Meth- ods： 30 clinical samples were selected randomly to observe the correlation of three methods;one normal and one abnormal sample were repeatedly measured ten times by automatic ESR analyzer in order to es- timate precision ;30 cases were chosen to be placed for 4.5 h, then measured every 30 rain by automat- ic ESR analyzer to observe the influence of placing time on the resuhs of test. Results：The results of ESR detected by automatic analyzer and Westergren method had a grate consistency （K〉~O. 81 ） , good correlation（r~〉0. 971 ）, right accuracy （ CV% ~〈7.70% ） in the normal group, and better accuracy （CV% 〈~4.94% ） in abnormal group. The results of Vital automatic ESR analyzer has a significant difference （P 〈 0.05） if the samples were placed for more than 3 hours, and ALIFAX Roller 20 auto- matic ESR analyzer has a significant difference （ P 〈 0.05 ） if the samples were placed for more than 1.5 hours. Conclusion： There may be a satisfactory consistency in detecting ESR by Vital automatic ESR analyzer method and ALIFAX Roller 20 automatic ESR analyzer method compared with Westergren method, but automatic ESR analyzer could shorten the test report time.
