经胸超声心动图对小儿心内膜弹力纤维增生症的诊断价值

摘　　要：目的探讨经胸超声心动图(transthoracic echocardiography,TTE)对小儿心内膜弹力纤维增生症(endocardial fibroelastosis,EFE)的诊断价值。方法选取我院2012年1月~2015年2月经临床确诊为EFE的38例患儿的TTE资料作为观察组,选取同时期38例健康儿童的TTE资料为对照组,分析TTE图像特点,重点观察患儿的房室内径、心内膜厚度、瓣膜反流及心功能各项指标等情况,将两组TTE检查结果进行对比分析。结果观察组患儿中,左心系统呈不同程度增大,其中左室增大22例,左房左室同时增大16例,室壁运动幅度弥漫性减低;左室心内膜呈不同程度增厚,厚度>3.5mm,回声增强;二尖瓣A峰峰值流速升高,而左室射血分数(LVEF)、左室短轴缩短率(FS)、每搏指数(SI)及心脏指数(CI)、E峰及E/A比值均较对照组减低,二者差异有统计学意义(P<0.05);二尖瓣伴有不同程度反流,其中轻度21例,中度8例,重度9例。结论左心扩大、左室心内膜增厚增强是EFE特征性TTE表现,通过对EFE患儿各项指标的评估,对患儿的早期诊断和评价预后有重要意义。Objective To investigate the diagnostic value of transthoraeic echocardiography （TTE） in children with endocar- dial fibroelastosis （EFE）. Methods TTE data of 38 children with EFE in our hospital from January 2012 to February 2015 were selected as the observation group and the TYE data of 38 healthy children in the same period were selected as as control group. Then, the characteristics of TTE image were analyzed, focusing on the children＇ s atrioventrieular diameter, intima - media thick- ness, heart valvular regurgitation, heart function indexes, and etc. Lasdy, TTE examination results were ana]yzed between the two group. Results In the observation group, the left ventricular system showed the enlargement at different degrees, in which 22 eases exhibited the left ventricular enlargement and 16 cases appeared the left ventricular and left atrial enlargement simultane- ously. Meanwhile, the diffuse wail motion decreased and the left ventricular endocardia showed thickness at different degrees, with thickness larger than 3.5 mm and echo enhancement. The mitral peak A velocity increased, while, left ventricular ejection fraction （LVEF）, left ventrieular fractional shortening （FS）, stroke volume index （SI） and cardiac index （CI）, peak E and E/ A ratio were all lower than those in control group, there was significant difference between the two groups （ P 〈 0.05 ）. The mi- tral regurgitation appeared at different degree, in which 21 cases were mild, 8 cases were moderate, and 9 cases were severe. Conduslon Left ventricular enlargement and left ventricular endocardial thickening are the characteristics of TIE manifestations of EFE, which are important to evaluate the early diagnosis and prognosis of children with EFE.
