自动测量方法测定Tei指数在评估法洛四联症胎儿左室功能中的应用研究

摘　　要：目的探讨自动测量和手动法获取法洛四联症(TOF)胎儿的左心室Tei指数(TI)在评估TOF胎儿左室功能中的应用价值。方法收集经胎儿超声心动图诊断为TOF的胎儿21例(病例组)和正常胎儿39例(对照组),采用左心室Tei指数(LVTI)自动测量法和分步手动测量方法,分析比较两种方法获得的LV-TI的差异性及两组胎儿LV-TI的变化趋势。结果运用TI自动测量方法获得的病例组LV-TI高于对照组(0.577±0.038vs 0.452±0.046),两组比较差异有统计学意义(t=10.59,P=0.00);两种测量方法获得的对照组及病例组胎儿LV-TI测量结果,差异无统计学意义(P=0.78、0.54)。结论 TI是较为理想评价TOF胎儿心功能的敏感指标,自动法较传统手动法测量更简单、快捷,具有更好可靠性和重复性,易于在实际及科研工作中推广应用。Objective To investigate the left function in fetuses with Tetralogy of Fallot （TOF） by Tel index （TI） using automatic measurement method. Methods Twenty one fetuses with TOF （case group） diagnosed by echo cardiography and 39 normal fetuses （control groups） were collected, Automatic measurement of left ventricular Tel index（LV TI） were achieved by using a measurement line placed in the aortic spectrum peak and by the manual measurement method of TI obtained step by step to obtain time intervals of the left ventricular inflow tract and outflow tract, The differences of LV-TI measured by the two methods and LV-TI between two groups were compared. Results The fetal LV-TI in case group was higher than that in control group by using TI automatic measurement methods （t= 10.59, P=0.00） ; There was no statistically difference between the two methods of the control group and case group （P=0. 78,0. 54）. Conclusions TI is sensitive index to assess fetal cardiac function with TOF, and TI automatic measurement method may he more simple and accurate than the traditional manual method, which could have a better reliability and repeatability and be easy to apply in clinical practice.
