卵巢内膜样囊肿腹腔镜手术后应用GnRH-a预防复发的临床研究

摘　　要：目的研究卵巢内膜样囊肿腹腔镜手术后使用GnRH-a治疗预防囊肿复发的临床效果。方法选取2013年3月-2016年3月在江阴市中医院就诊治疗的卵巢内膜样囊肿患者60例,征得患者和其家属的同意后随机分为对照组和试验组各30例。两组患者均予以腹腔镜手术进行治疗,试验组在术后使用GnRH-a治疗,随访1年,观察比较两组患者术后卵巢功能、复发率、妊娠率以及治疗效果。结果试验组患者治疗总有效率90.0%明显高于对照组的70.0%,差异有统计学意义(P<0.05)。术后6个月试验组患者卵巢功能各指标均低于对照组,随访1年妊娠率高于对照组,囊肿复发率低于对照组,两组比较差异均有统计学意义(P<0.05)。结论卵巢内膜样囊肿经腹腔镜手术后使用GnRH-a治疗,可提高治疗效果,有效改善患者血清性激素水平,提高妊娠率,预防和减少囊肿的复发。Objective To research the preventive effect of gonadotropin-releasing hormone angonist （GnRH-a） on relapse of ovarian endometrioid cyst after laparoscopic surgery. Methods Sixty patients with ovarian endometrioid cyst diagnosed and treated in Jiangyin Hospital of Traditional Chinese Medicine from March 2013 to March 2016 were selected and randomly divided into control group and experimental group after receiving the consent of the patients and their families, 30 patients in each group. The patients in the two groups were treated by laparoscopic surgery, and the patients in experimental group were treated by GnRH-a after laparoscopic surgery. The patients in the two groups were followed up for one year. The postoperative ovarian function, recurrence rates, pregnancy rates, and curative effects in the two groups were observed and compared. Results The total effective rate in experimental group was 90. 0% , which was statistically significantly higher than that in control group （70. 0% ） （P〈0. 05） . The indexes of ovarian function at 6 months after laparoscopic surgery in experimental group were statistically significantly lower than those in control group, the pregnancy rate at one year after laparoscopic surgery in experimental group was statistically significantly lower than that in control group, the recurrence rate in experimental group was statistically significantly lower than that in control group （P〈0. 05） . Conclusion GnRH-a can improve therapeutic effect, effectively increase the levels of serum sex hormones and pregnancy rate, prevent relapse and reduce the recurrence rate in treatment of ovarian endometrioid cysts after laparoscopic surgery.
