悬饮方+尿激酶治疗结核性包裹性胸腔积液疗效及对炎性因子、腺苷酸脱氢酶、乳酸脱氢酶的影响

摘　　要：目的探讨悬饮方+尿激酶治疗结核性包裹性胸腔积液疗效及对炎性因子、腺苷酸脱氢酶、乳酸脱氢酶的影响。方法将80例结核性包裹性胸腔积液患者随机分为2组,对照组40例给予尿激酶治疗,观察组40例在此基础上加用悬饮方治疗,2组治疗时间均为2周。观察2组胸腔液体抽吸总量、胸液吸收时间、胸膜厚度及治疗前后中医证候积分、C反应蛋白、肿瘤坏死因子-α、白细胞介素-8、腺苷酸脱氢酶及乳酸脱氢酶水平变化情况,统计2组近期疗效和不良反应发生情况。结果观察组胸腔液体抽吸总量明显多于对照组(P<0.05),胸液吸收时间明显短于对照组(P<0.05),胸膜厚度显著低于对照组(P均<0.05);2组治疗后寒热往来、身热起伏、咳嗽痰少、胸胁刺痛、舌苔薄白或黄及脉弦积分和血清C反应蛋白、肿瘤坏死因子-α、白细胞介素-8、腺苷酸脱氢酶及乳酸脱氢酶水平均显著降低(P均<0.05),且观察组治疗后各项指标水平均显著低于对照组(P均<0.05);观察组近期治疗总有效率显著高于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论悬饮方+尿激酶治疗结核性包裹性胸腔积液可有效缓解全身症状体征,抑制机体炎症反应,下调腺苷酸脱氢酶和乳酸脱氢酶水平,且安全。Objective It is to investigate the curative effect of Xuanyin Decoction plus urokinase in the treatment of tuberculous encapsulated pleural effusion and its influence on inflammatory factors,adenylate dehydrogenase and lactate dehydrogenase. Methods 80 patients with tuberculous encapsulated pleural effusion were randomly divided into 2 groups,40 cases in the control group were treated with urokinase; on the basis of this,40 cases in the observation group were treated with the decoction of suspensions. The two groups were treated for 2 weeks. The total volume of fluid suction,pleural fluid absorption time and pleura thickness of 2 groups were observed before and after treatment,and the changes of TCM syndrome integral,C reactive protein,tumor necrosis factor alpha,interleukin-8,adenylate dehydrogenase and lactate dehydrogenase level before and after treatment were observed. The short-term effects and adverse reactions of two groups were observed. Results The total amount of liquid aspiration in the thoracic cavity of the observation group was more than that of the control group（ P 0. 05）,the absorption time of the pleural fluid and the thickness of the pleura were significantly lower than that of the control group（ all P 0. 05）. After treatment,the integral of fever and chill alternate attack,fever fluctuate,cough with little sputum,pricking pain in the chest,thin and white or yellow coating on the tongue,stringy pulse,and the levels of serum C reactive protein,tumor necrosis factor alpha,interleukin-8,adenosine monophosphate dehydrogenase and lactate dehydrogenase were significantly decreased（ all P 0. 05）,and all the above indicators of the observation group were significantly lower than those of the control group（ all P 0. 05）. The total effective rate of the observation group was significantly higher than that of the control group（ P 0. 05）.
