左旋卡尼汀联合益气活血汤对冠心病心肌缺血患者血流动力学的影响

摘　　要：目的观察左旋卡尼汀联合益气活血汤对冠心病心肌缺血患者血流动力学的影响。方法将74例冠心病心肌缺血患者随机分为2组,对照组37例应用左旋卡尼汀治疗,研究组37例应用左旋卡尼汀联合益气活血汤治疗,2组均持续治疗4周。评定2组治疗后临床疗效,统计2组治疗前后心率、收缩压和舒张压,检测2组治疗前后心功能指标以及血流动力学指标。结果治疗后,研究组治疗总有效率显著高于对照组(P均<0.05);2组心率、收缩压和舒张压均较治疗前显著降低(P均<0.05),且研究组均显著低于对照组(P均<0.05);2组收缩期时间(Ts)、收缩期速度时间积分(S-VTI)和收缩期峰值速度(Vs)均较治疗前显著升高(P均<0.05),且研究组均显著高于对照组(P均<0.05);2组全血黏度高切值、全血黏度低切值和血浆比黏度均较治疗前显著降低(P均<0.05),且研究组均显著低于对照组(P均<0.05)。结论左旋卡尼汀联合益气活血汤治疗冠心病心肌缺血患者疗效显著,能明显改善患者血流动力学,增强心功能。Objective It is to observe the effects of L-carnitine and Yiqi Huoxue Decoction on hemodynamics in patients with coronary heart disease and myocardial ischemia. Methods 74 cases of patients with coronary heart disease with myocardial ischemia were randomly divided into two groups,37 cases in the control group were treated with L-carnitine,37 cases in the study group were treated with L-carnitine combined with Yiqi Huoxue Decoction,and both groups were treated for 4 weeks. The clinical effects of two groups were evaluated after treatment. The heart rate,systolic pressure and diastolic blood pressure were measured before and after treatment in the two groups. The cardiac function indexes and hemodynamic indexes were measured before and after treatment. Results After treatment,the total effective rate of the study group was significantly higher than that of the control group( P < 0. 05). The heart rate,systolic pressure and diastolic pressure of the study group after treatment were significantly lower than those before treatment( P < 0. 05),and these indexes in the study group were significantly lower than those in the control group( P < 0. 05). The systolic time( Ts),systolic velocity integral( S-VTI) and systolic peak velocity( Vs) of the two groups after treatment were significantly higher than those before treatment( P < 0. 05),and these indexes in the study group were significantly higher than those in the control group( P <0. 05). The high whole blood viscosity,low whole blood viscosity and plasma specific viscosity of the two groups were significantly lower after treatment than those before treatment( P < 0. 05),and these indexes were significantly lower in the study group than those in the control group( P < 0. 05). Conclusion L-carnitine combined with Yiqihuoxue Decoction has significant curative effect on myocardial ischemia,and can significantly improve the patient's hemodynamics.
