助产士主导模拟分娩教育及自我效能增强干预在初产妇中的应用

摘　　要：目的:探讨助产士主导模拟分娩教育及自我效能增强干预在初产妇中的应用效果。方法:选择我院2016年1~12月建卡的初产妇223例,随机分为观察组104例和对照组119例,对照组接受门诊常规产检,观察组在助产士主导下进行一系列产前干预,包括模拟分娩教育及自我效能增强干预等,入组时调查两组对分娩方式的选择,比较两组最终分娩方式、各产程时间、分娩疼痛评分。结果:观察组自然分娩率高于对照组(P<0.05),无剖宫产指征剖宫产率低于对照组(P<0.05),第一产程和总产程时间短于对照组(P<0.05),疼痛程度轻于对照组(P<0.05)。结论:助产士主导模拟分娩教育及自我效能增强干预可帮助初产妇树立自然分娩的信心,减少社会因素导致的剖宫产,有效地缓解疼痛并改善分娩结局。Objective： To explore the application effect of simulation childbirth education and self-efficacy enhancement intervention in primipara led by midwives. Methods： To select 223 primiparas who had registered for a card in our hospital from Jan. to Dec. 2016,randomly divide them into the observation group 104 and the control group 119. The control group received the antenatal care,and the observation group conducted a series of antenatal interventions under the guidance of the midwives,including simulation childbirth education and self-efficacy enhancement intervention. The selection of childbirth mode,the final childbirth mode,duration of childbirth and the score of pain degrees were compared between the two groups. Results： It was found that that natural birth rate of the observation group was higher than that of the control group（ P 〈0. 05）,the cesarean section without medical indication was lower than that of the control group（ P〈 0. 05）,and the first stage of labor and total stage of labor were shorter than that of the control group（ P 〈0. 05）,and the pain degrees were less than that of the control group（ P 〈0. 05）. Conclusion： The application of simulation childbirth education and self-efficacy enhancement intervention led by midwife in primipara that was helpful to primipara to establish the confidence of natural childbirth,reduce the cesarean section induced by social factors,effectively relieve pain and improve the outcome of childbirth.
