北京成人腹泻患者星状病毒感染的分子流行病学特征

摘　　要：的 了解北京市成人腹泻患者星状病毒感染的分子流行病学特征.方法 收集2008年8月至2009年7月首都医科大学宣武医院519份14岁以上腹泻患者的腹泻粪便样本和相关病例临床信息,利用逆转录-聚合酶链式反应（reverse transcription-polymerase chain reaction,RT-PCR）进行星状病毒检测,对星状病毒阳性PCR产物测序并进行系统进化分析.结果 星状病毒检出的阳性率为6.7％（35/519）,其中男性为15例,阳性率为5.6％ （15/270）;女性为20例,阳性率为8.0％（20/249）,差异无统计学意义（x2=1.26,P=0.261）;季节高峰出现在10月（17.5％,14/80）;在≥60岁的年龄组中检出率最高（8.9％,7/79）.对35份星状病毒阳性标本进行测序,获得24株序列,系统进化分析结果显示,17株（70.8％,17/24）为经典的HAstV-1型星状病毒,同时检测到4株HAstV-2型星状病毒,1株HAstV-4型星状病毒,1株HAstV-5型星状病毒,1株HAstV-8型星状病毒.结论 星状病毒是北京市成人腹泻的重要病原体之一,HAstV-1型是流行优势株.Objective To investigate the molecular epidemiological characteristics of human astrovirus （HAstV） in adults with diarrhea in Beijing,China.Methods A total of 519 fecal specimens from patients ≥14 years of age with acute gastroenteritis were collected with relevant clinical data in Xuanwu Hospital Capital Medical University from August 2008 to July 2009.HAstV was detected by RT-PCR,positive PCR products were sequenced and phylogenetic analyses were carried out.Results The total positive rate of HAstV was 6.7% （35/519）,of which 15 were male,the positive rate was 5.6% （15/270）;the female was 20 cases,the positive rate was 8.0% （20/249）.There was no statistically significant difference （x2=1.26,P=0.261）.The seasonal peak occurred in October （17.5%,14/80） and the highest detection rate was in the age group of≥ 60 （8.9%,7/79）.Positive specimens were sequenced and 24 sequences were obtained.Phylogenetic analysis showed that seventeen （70.8%,17/24） strains were classic HAstV-1,four strains were HAstV-2,one strain was HAstV-4,one strain was HAstV-5 and one strain was HAstV-8.Conclusions HAstV is one of the important pathogens causing diarrhea in adults in Beijing and classic HAstV-1 is the predominant strain.
