等离子髓核成形术治疗椎间盘源性腰腿痛的临床观察

摘  要:目的:探讨等离子髓核成形术治疗椎间盘源性腰腿痛的近期临床疗效。方法:2014年9月~2016年9月对52例具有临床症状的榷间盘源性腰腿痛患者,采用等离子髓核成形术治疗,采用Macnab、视觉疼痛模拟评分(VAS)对患者术后主观满意度、腰腿疼缓解情况进行评估,并记录患者平均住院日及手术并发症。结果:本组患者随访6—12个月,平均10个月,术前、术后7天、术后1个月及术后末次随访时VAS分值分别为7.93±0.71分、3.14±0.65分、2.41±0.54分、2.95±0.79分。术后7天、术后1个月及术后末次随访时VAS分值与术前比较,差异有统计学意义(P〈0.05);术后7天、术后1个月及术后末次随访时VAS分值间比较,差异无统计学意义(P〉0.05),改良Macnab法分析术后疗效优良率为82.69%,患者平均住院日5±1.25天,未发现手术相关并发症。结论:等离子髓核成形术治疗椎间盘源性腰腿痛的近期疗效肯定,且这种技术简单、微创、安全,对其远期疗效,仍需进一步研究。Objective: : To evaluate the curative effect of coblation nucleoplasty treatment for the discogenic low back pain. Methods: A total of 52 patients with discogenic low back pain were treated by coblation nucleoplasty during September 2014 to September 2016. Pa- tient's subjective satisfaction, relief of low back pain or leg pain after operation were assessed by Maenab and Visual Analogue pain Scale ( VAS), and the patientg average hospital day and surgical complications were also recorded. Results: All patients were followed - up for 6 - 12 months, the average of 10 months. VAS respectively were(7.93 ± 0.71 ), (3.14 ±0.65), (2.41 ±0.54), (2.95 ±0.79)at pre - operation,at 7 - days post - operation,at 1 - month post - operation and at the last follow - up. Compared with the pre - operative, the VAS score difference was statistically significant with7 - days post - operation, 1 - month post - operation and at the last follow - up ( P 〈 0.05 ). There were no statistically significant difference of VAS score between 7 - days post - operation, 1 - month post - operation and at the last follow - up ( P 〉 0.05 ). Conclusion: Coblation nucleoplasty may have satisfactory recent clinical outcomes for treatment the diseogenic low back pain. The technique should be simple, minimally invasive and safe, and further study should be needed for its long - term efficacy.
