摘 要:目的研究COPD患者CT血管成像结果与患者肺功能参数、血气分析结果的相关性。方法选择2014年3月-2016年10月在本院就诊的132例慢性阻塞性肺疾病患者,根据慢性阻塞性肺疾病全球倡议(GOLD)分级分为1级~4级,行胸部CT血管成像并测定左侧支气管动脉管径、右侧支气管动脉管径,行肺功能检测、血气分析以及血清炎性因子含量测定。结果不同GOLD分期COPD患者的支气管动脉管径、肺功能参数、血气分析指标、血清炎性因子含量差异有统计学意义(P<0.05);GOLD分期越高,左侧支气管动脉管径、右侧支气管动脉管径、Pa CO2以及血清TNF-α、IL-6、IL-8含量越高,FEV1/FVC、MEF25、MEF50、MEF25-75以及Pa O2越低,两两比较差异均有统计学意义(P<0.05)。Pearson相关性分析显示,左侧和右侧支气管动脉管径与FEV1/FVC、MEF25、MEF50、MEF25-75以及Pa O2呈负相关,与Pa CO2以及TNF-α、IL-6、IL-8含量呈正相关(P<0.05)。结论 COPD患者CT血管成像测定支气管动脉管径与肺功能参数、血气分析具有良好的相关性,能够用于COPD患者肺功能的评估。Objective To study the correlation of CT angiography results with pulmonary function parameters and blood gas analysis result in patients with chronic obstructive pulmonary disease( COPD). Methods 132 patients with chronic obstructive pulmonary disease in our hospital during March 2014-October 2016 were selected and divided into grade 1-4 according to GOLD grade. Chest CT angiography was performed,and left bronchial artery diameter,right bronchial artery diameter were determined.Pulmonary function test,blood gas analysis and serum inflammatory factor determination were performed. Results Differences of bronchial artery diameter,pulmonary function parameters,blood gas analysis indexes and serum inflammatory factor in COPD patients with different GOLD stages had statistical significance( P〈 0. 05); the higher the GOLD stage was,the higher the left bronchial artery diameter,right bronchial artery diameter,PaCO_2 and serum TNF-α,IL-6,IL-8 contents were,and the lower the FEV1/FVC,MEF25,MEF50,MEF25-75 and PaO_2 were,differences between two of them were statistically significant( P 〈0. 05).Pearson correlation analysis showed that left and right bronchial artery diameters were negatively correlated with FEV1/FVC,MEF25,MEF50,MEF25-75 and PaO_2,positively correlated with PaCO_2 and serum TNF-α,IL-6,IL-8 contents( P 〈0. 05).Conclusion Bronchial artery diameter determined by CT angiography has a good correlation with pulmonary function parameters and blood gas analysis,and it can be used for the evaluation of pulmonary function in COPD patients.
