摘 要:目的探讨肥胖与非肥胖型多囊卵巢综合征代谢特征(PCOS)。方法 204例多囊卵巢综合征患者按体质指数(BMI)分为肥胖组59例和非肥胖组145例,对患者的糖、脂代谢指标进行测定分析。结果肥胖组和非肥胖组胰岛素抵抗(IR)的发生率分别为61.02%(36/59)和39.31%(57/145),两组比较,差异有统计学意义(P<0.05);肥胖组和非肥胖组高胰岛素血症的发生率分别为35.59%(21/59)和22.07%(32/145),两组比较,差异有统计学意义(P<0.05)。肥胖组和非肥胖组糖耐量受损(IGT)发生率分别为20.34%(12/59)和11.03%(16/145),两组比较,差异有统计学意义(P<0.01)。肥胖组和非肥胖组糖尿病发生率分别为6.78%(4/59)和2.07%(3/145),差异有统计学意义(P<0.01)。肥胖组较非肥胖组空腹胰岛素及服糖后30min、120min血糖及胰岛素明显升高,差异有统计学意义(P<0.01)。肥胖组较非肥胖组甘油三酯明显升高,差异有统计学意义(P<0.05)。总胆固醇、高密度脂蛋白、低密度脂蛋白及空腹血糖,两组比较差异无统计学意义(P>0.05)。结论 PCOS肥胖者较非肥胖者更易发生糖、脂代谢的紊乱。Objective To investigate metabolic characteristics of obese Women and non-obese women with PCOS. Methods The metabolic index of 204 cases with PCOS,which were divided into 59 cases in obese and 145 cases in non-obese were analyzed and compared. Results The rates of insulin resistance were 61. 02%( 36/59) in obese group and 39. 31%( 57/145) in non-obese group,which showed significant difference( P〈0. 05). The rates of hyperinsulinemia were 35. 59%( 21/59) in obese group and 22. 07%( 32/145) in non-obese group,which showed significant difference( P〈0. 05). The rates of impaired glucose tolerance( IGT) were 20. 34%( 12/59) in obese group and 11. 03%( 16/145) in non-obese group,which showed significant difference( P〈0. 01). The rates of diabetes mellitus were6. 78%( 4/59) in obese group and 2. 07%( 3/145) in non-obese group,which also showed significant difference( P〈0. 01). At the time of30 min and 120 min,the levels of glucose and at the time of fasting,30 min and 120 min the levels of insulin were higher in obese group than in non-obese group,which all showed statistical difference,at every time point( P〈0. 01). The levels of triglyceride in the obese group was greatly higher than in non-obese group,with a significant difference( P〈0. 05). There was no significant difference between the two groups in terms of TC,HDLC,LDLC and FBG. Conclusion Abnormal glucose and lipid metabolism are observed more frequently in obese PCOS women than non-obese PCOS women.
