自拟方补肾通络汤治疗老年特发性肺纤维化的临床研究

摘　　要：目的观察补肾通络汤对老年特发性肺纤维化患者肺功能、转化生长因子及炎症因子的影响。方法将符合诊断标准的老年特发性肺纤维(IPF)患者62例,采用随机分组的方法分为对照组、治疗组,对照组32例给予规范的西医治疗方案,治疗组30例在对照组治疗的基础上,给予补肾通络汤口服。两组治疗12周后检测肺功能(FEV1L、FVC/L、FEV1/FVC/%)、6 min步行实验、转化生长因子(TGF-β1)、及炎症因子(CRP和TNF)进行指标比较。结果治疗后治疗组总有效率为97.0%,对照组总有效率为62.5%,两组比较差异有统计学意义(P<0.05);两组患者的肺功能(FEV1L、FVC/L、FEV1/FVC/%)、转化生长因子(TGF-β1)、炎症因子(CRP和TNF)指标与治疗前比较均明显改善,差异有统计学意义(P<0.05),且治疗组均优于对照组,差异有统计学意义(P<0.05);治疗组6 min步行实验与治疗前比较,差异有统计学意义(P<0.05)。结论补肾通络汤治疗老年特发性肺纤维化可改善患者的肺功能以及步行长度,提高患者生活质量,其作用机制与抑制炎症因子(CRP和TNF)、阻断转化生长因子(TGF-β1)作用有关。Objective To observe the effects of bushen tongluo decoction on pulmonary function,transforming growth factor（ TGF） and inflammatory factors in the patients with senile idiopathic pulmonary fibrosis（ IPE）. Methods A total of 62 patients met the diagnostic criteria of IPF were randomized into a control group and a treatment group. In the control group（ 32 cases）,the scheme of western medicine was specified. In the treatment group（ 30 cases）,based on the treatment as the control group,bushen tongluo decoction was given for oral administration. In 12 weeks of treatment,the pulmonary functions（ FEV1 L,FVC/L,FEV1/FVC/%）,6-min walk test,TGF-β1 and the inflammatory factors（ CRP and TNF） were compared between the two groups. Results After treatment,the total effective rate was 97. 0% in the treatment group and was62. 5% in the control group,indicating the significant differences（ P〈0. 05）. The pulmonary functions（ FEV1 L,FVC/L,FEV1/FVC/%）,TGF-β1 and the inflammatory factors（ CRP and TNF） were all remarkably improved after treatment as compared with those before treatment,indicating the significant differences（ P〈0. 05）. The improvements in the treatment group were superior to the control group,indicating the significant difference（ P〈0. 05）. The result of 6-min walk test in the treatment group was different significantly as compared with that before treatment（ P〈0. 05）. Conclusion Bushen tongluo decoction improves the pulmonary functions,increase walking distance and improves living quality of the patients with IPF. The effect mechanisms are related to the inhibition of inflammatory factors（ CRP and TNF） and the blockage of TGF-β1.
