浅析扩张后随意型皮瓣修复面颈部烧伤后瘢痕的临床效果

摘　　要：目的了解对面颈部烧伤患者存在瘢痕用扩张后随意型皮瓣修复手段进行治疗的干预效果。方法分析对象来自本文所在医院2012年-2017年期间面颈部遭受烧伤且留下瘢痕患者126例,所有患者应用扩张后随意型皮瓣修复手段治疗,在其附近埋置皮肤软组织扩张囊,将继发创面切除,对患者相关资料进行回顾性分析并归纳扩张后随意型皮瓣修复面颈部烧伤后瘢痕的临床治疗效果。结果对所有面颈部烧伤后瘢痕患者放置230个皮肤软组织扩张囊,通过扩张后随意型皮瓣修复方案顺利将继发创面切除,其中包括滑行推进皮瓣、旋转皮瓣、易位皮瓣、复合转移皮瓣,个数分别为90个、64个、24个、52个。治疗后出现相关并发症患者例数20例,包括皮瓣下血肿、皮瓣血运障碍以及刀口感染,例数分别为10个、8个、2个,对其进行对症处理后顺利治愈,对疗程及效果并无影响。结论脸部或者颈部烧伤后留下瘢痕患者用扩张后随意型皮瓣修复手段进行治疗能够取得令人满意的瘢痕修复效果,修复后瘢痕和临近位置在色泽、质地等方面都和正常皮肤相近,有突出的治疗价值,值得临床推广。Objective To investigate the effect of the treatment of the patients with burn in the neck and neck after the repair of scar with expanded random skin flap. Methods To analyze the object from the hospital during the period 2012-2017 suffered burns of face and neck and left scar in 126 cases with free flap treatment of all patients after dilatation in the vicinity of the buried skin and soft tissue expander,the secondary wound scar resection, clinical curative effect to patients data were retrospectively analyzing and summarizing the expanded random pattern skin flap in face and neck after burn. Results To place the 230 skin and soft tissue of patients with scar all face and neck after burn the expander through expanded random flap repair scheme will smooth the secondary wound excision,including sliding flap,rotation flap,flap translocation,composite flap transfer,the number were 90,64,24,52.After the treatment of 20 cases of complications related to the number of patients,including subcutaneous hematoma,flap blood circulation disorders and wound infection,the number of cases were 10,8,2,symptomatic treatment after the cure,there is no impact on the course and effect. Conclusion The face or neck after burn scars with expanded flap repair means of treatment can achieve satisfactory effect of scar repair,repair scar and adjacent position in color,texture and so on are similar to that of normal skin,treat the value of outstanding,worthy of promotion.
