门诊处方点评及合理用药情况思考

摘　　要：目的通过对我院门诊处方的点评及合理用药情况的思考,为提高我院处方质量和合理用药水平提供参考依据。方法随机选取我院2015年1月至2016年12月的门诊处方2000张,按年进行点评,其中包括2015年1~12月门诊处方1000张,2016年1~12月门诊处方1000张,然后按照处方点评标准对其进行点评,以及对用药情况进行分析。结果 2015年1至12月至2016年1~12月我院处方合格率由89.45%明显提升至95.53%(P<0.05),其中不合理的处方主要体现在:处方书写不规范、药品剂型书写不规范、给药途径不适宜、用法用量不适宜、用药不适宜等。结论通过对门诊处方的的点评,能及时发现门诊处方存在的不合理现象,加强药剂师与临床医生的沟通,并提出合理的建议,结果显示处方点评能明显提高门诊处方质量及合理用药水平。Objective Through thinking of outpatient prescription in our hospital and review the rational use of drugs, to provide refcrencc for improving the quality of prescription in our hospital and the level of rationaI drug use. Methods Randomly selected in our hospital from January 2015 to December 2016 a total of 2000 outpatient prescriptions, annual reviews, including 2015 1 to December 1000 outpatient prescriptions in 2016 1. To December 1000 outpatient prescriptions, and then comments on the prescription and in accordance with the standards of drug use were analyzed. Results In 2015 1 to December to 2016 1 to December by the qualified rote of prescription in our hospital The 89.45% was increased to 95.53% （P〈0.05）, the unreasonable prescription is mainly reflected in： prescription writing is not standardized, drug dosage form of writing is not standardized, the route of administration is not appropriate, the amount of usage is not appropriate, inappropriate medication. Conclusion The outpatient prescription comment, can discover the unreasonable phenomenon of outpatient prescription there the pharmacists and clinicians to strengthen communication, and put forward reasonable suggestions, results show that the prescription can significantly improve the level of quality of prescription and rational drug use in clinic.
