大隐静脉高位结扎加点状剥脱联合泡沫硬化剂治疗下肢静脉曲张的临床应用研究

摘　　要：目的探讨大隐静脉高位结扎加点状剥脱联合泡沫硬化剂治疗下肢静脉曲张临床疗效。方法对我院行该术式治疗的107例下肢静脉曲张,对大腿部和膝下8cm主干行高位结扎加剥脱,对大隐静脉分支直径大于6cm行点状剥脱,小于或等于6cm行泡沫硬化剂注射治疗。归纳总结其临床和随访资料,从而得出结论。结果所有患者均顺利出院,平均出血量(35.1±16.4m L)、平均切口数(3±1.6个)、手术耗时(40.2±25.5min),11例患者大腿沿大隐静脉走形出现皮下硬结,其中3例患者出现大腿皮下瘀斑,1例患者出现伤口少量渗血;10例患者出现泡沫硬化剂注射血管血栓性浅静脉炎,8例给予硫酸镁外敷好转,2例给予切开排出血栓后好转;术后随访1年,无1例患者复发。结论本手术方式结合传统大隐静脉高位结扎和点状剥脱的优点对主干和大的分支行彻底剥脱,并结合微创要求,行小的分支泡沫硬化剂治疗,临床效果佳,手术相对微创,值得推广。Objective To investigate the clinical efficacy of high saphenous vein ligation and point-like exfoliation combined with foam sclerotherapy in the treatment of varicose veins of lower extremities. Methods 107 cases of lower extremity varicose veins treated by our hospital were treated with high ligation and dissection of the trunk of the thigh and knee under 8cm. Conclusion The combination of traditional high saphenous vein ligation and punctat; exfoliation of the traditional trunk and large branches of the line completely stripped, combined with minimally invasive requirements, a small branch of the foam curing agent treatment, clinical effect is good, relatively small surgery Record, worthy of promotion.
