摘　　要：目的了解德州市儿童含麻疹成分疫苗(Measles-containing Vaccine,MCV)及时接种情况及其影响因素,为制定相应的管理措施提供依据。方法采用随机整群抽样方法,调查德州市11个县市区和经济开发区1 119名儿童第一、二剂MCV免疫接种情况、家庭基本状况、家长认知等相关情况,采用Logistic多因素分析方法分析影响儿童及时接种率的主要因素。结果调查1 119名儿童,其中男童620名(占55.41%),女童499名(占44.59%);常住儿童1 015名(占90.70%),流动儿童104名(占9.30%)。MCV接种率为95.26%,及时接种率为78.73%,常住儿童的及时接种率(81.97%)高于流动儿童(47.15%)(P<0.01);MCV第一剂次接种率(96.60%)低于第二剂次(96.70%)(P>0.05),第一剂次及时接种率(81.23%)低于MCV第二剂次(92.25%)(P<0.01);因病延误(65.97%)是儿童MCV不及时接种的主要原因,其次为儿童流动(16.39%)和担心疫苗安全(5.04%)。Logistic多因素分析显示:母亲文化程度、家庭年经济收入、家长对接种含麻疹成分疫苗的态度与认知情况、儿童流动是影响MCV及时接种的主要因素。结论德州市儿童含麻疹成分疫苗及时接种率较低。Objective To understand the timely coverage rate of measles-containing vaccine immunization and related factors among children in Dezhou city,so as to provide basis for working out relevant management measures. MethodsThe cluster sampling method was used and 1 119 children from 11 counties and economic development zone of Dezhou were surveyed for their status of first dose and the second dose measles-containing vaccine immunization and their basic status of the family and parent cognition related to the immunization.The multi-variate Logistic analysis was used to analyze the main factors affecting children＇s timely vaccination. Results A total of 1 119 children were surveyed,including620 boys（55.41%）and 499 girls（44.59）;including 1 015 permanent children（90.70%）and 620 migrant children（9.30%）.The coverage rate of measles-containing vaccine（MCV）was 95.26%,and the timely coverage rate was 78.73%.The rate of timely vaccination for local children（81.97%）was higher than that（47.15%）of floating children（P〈0.01）.The coverage rate of the first dose（96.60%）was lower than that of the second dose（96.70%）（P〈0.05）,the first dose timely coverage rate of MCV（81.23%）was lower than that of the second dose timely coverage rate of MCV（92.25%）,（P〈0.01）.The main reasons for not timely vaccination were delayed vaccination due to illness（65.97%）,child migration（16.39%）and concerns about vaccine safety（5.04%）.The results of multi-variate Logistic analysis suggested that the main factors that could influence the timely coverage rate of MCV were mother＇s education level,annual income of families,parents＇ attitudes and perceptions of vaccination,the transfer of living area. Conclusion The timely coverage rate of measles-containing vaccine immunization is relatively low among children in Dezhou city.
