SATB2在胃肠腺癌中的表达及其诊断价值

摘　　要：目的检测SATB2在胃肠道腺癌中的表达并探讨其诊断价值。方法采用免疫组化Envision法检测92例结直肠腺癌及69例胃腺癌中SATB2、CDH17和CDX-2蛋白的表达情况。结果 (1)SATB2、CDH17和CDX-2在结直肠腺癌中的阳性率分别为81.5%~82.6%、94.6%和96.7%;SATB2、CDH17和CDX-2在胃腺癌中的阳性率分别为8.7%~10.1%、5.9%和78.2%,3种抗体在结肠癌和胃癌之间的表达差异均显著(P<0.05)。(2)SATB2、CDH17和CDX-2在结直肠癌与胃癌鉴别诊断中的阳性预测值是81.5%~82.6%、94.6%和96.7%,阴性预测值是89.9%~91.3%、40.6%和21.7%。(3)3种抗-SATB2单克隆抗体均显示细胞核着色,3种抗体的检测结果具有较高的一致性(κ=0.93)。结论 SATB2在结直肠癌中的表达明显高于胃腺癌中的表达,在结直肠癌与胃腺癌鉴别诊断中优于CDX-2和CDH17。Objective To detect the expression of SATB2 in gastrointestinal adenocarcinoma and its diagnostic value.Methods The expression of SATB2,CDX-2 and CDH17 was detected using immunohistochemical En Vision two-step method in 92 cases of colorectal carcinoma and 69 cases of gastric carcinoma. Results The positive rates of SATB2,CDX-2 and CDH17 in colorectal carcinoma were 81. 5% ~ 82. 6%,94. 6% and 96. 7%; the positive rates of SATB2,CDX-2 and CDH17 gastric carcinoma were 8. 7% ~ 10. 1%,59. 4% and 78. 2%,respectively; the expression of all three markers showed significance difference between colorectal carcinoma and gastric carcinoma（ P 0. 05）. The positive predictive value of SATB2,CDX-2 and CDH17 in the differencial diagnosis of colorectal carcinoma and gastric carcinoma was 81. 5%-82. 6%,94. 6% and 96. 7%,respectively; the negtive predictive value was 89. 9%-91. 3%,40. 6% and21. 7%,respectively. All of the three anti-SATB2 antibody showed nuclear immunoreactivity,the repeatabilites of positive rate and staining intensity were better among all the three anti-SATB2 monoclonal antibodies（ κ = 0. 93）. Conclusion The expression of SATB2 in colorectal carcinoma is significantly high than that in gastric carcinoma,which is prior to CDX-2 and CDH17 in the differencial diagnosis of colorectal carcinoma and gastric carcinoma.
