摘  要:目的:观察柴胡疏肝汤对炎症诱导的肠易激综合征(irritable bowel syndrome,IBS)大鼠下丘脑-垂体-肾上腺皮质(hypothalamic-pituitary-adrenal,HPA)轴亢进和内脏高敏的影响。方法:48只Wistar大鼠随机分为对照组,模型组,匹维溴铵组,柴胡疏肝汤低剂量组、中剂量组和高剂量组,采用乙酸灌肠法造模。造模后第7周,予匹维溴铵和柴胡疏肝汤治疗2周,检测血清IL-1β、IL-6水平,结肠5-HT含量及促肾上腺皮质激素释放激素受体1和2(corticotropin releasing hormone receptor 1 and 2,CRH-R1和CRHR2)mRNA的表达,地塞米松/促肾上腺皮质激素释放激素测试各阶段的皮质酮(Corticosterone,CORT)水平,评价内脏高敏情况。结果:肠易激综合征大鼠模型出现IL-6、IL-1β水平升高,地塞米松/促肾上腺皮质激素释放激素测试不同时间段CORT水平升高,结肠5-HT含量增加,CRHR1 mRNA的表达上调,回缩反射中压力阈值降低,而避水应激实验中粪粒数明显增加。柴胡疏肝汤(5、10g/kg)下调肠易激综合征大鼠模型IL-6、IL-1β水平,降低地塞米松/促肾上腺皮质激素释放激素测试不同时间段CORT水平,减少结肠5-HT含量,抑制结肠CRH-R1 mRNA的表达,正常化回缩反射中压力阈值和减少避水应激实验中粪粒数。结论:柴胡疏肝汤通过下调IL-6、IL-1β水平,纠正失调的HPA轴,减少结肠5-HT含量,抑制结肠CRHR1 mRNA的表达,降低大鼠肠道敏感性而治疗肠易激综合征。Objective: To investigate the effect of chaihu shugan decoction( CHSGD) on hypothalamic-pituitary-adrenal( HPA) axis hyperfunction and the visceral hypersensitivity which were induced by inflammation in rats with irritable bowel syndrome( IBS). Methods: Forty rats were randomly divided into four groups,including the control group,the model group,pinaverium group,CHSGD high,medium and low doses of groups. Model rats were made by acetic acid enemata. Seven weeks after acetic acid enemata,rats received pinaverium and CHSGD treatment for 2 weeks. Serum IL-1β and IL-6 expressions,colon 5-HT contents,colon corticotropin releasing hormone receptor 1 and 2( CRH-R1 and CRH-R2) mRNA were detected. Plasma corticosterone( CORT) was detected in different phases of dexamethasone/corticotropin releasing hormone( DEX/CRH) test and the visceral hypersensitivity was evaluated. Results: In the model group,colon 5-HT content,levels of serum IL-1β and IL-6 were elevated,as well as plasma CORT levels in different phases of DEX/CRH test( all p 0. 01); the pressure threshold of abdominal withdrawal reflex( AWR) was decreased and the amount of defecation in avoid-water stress test was increased( all p 0. 01). CHSGD( 5 g/kg,10 g/kg) lowered expressions of serum IL-1β and IL-6 and colon 5-HT contents,inhibited colon CRH-R1 mRNA,downregulated plasma CORT levels in different phases of DEX/CRH test,normalized the pressure threshold of AWR and decreased the amount of defecation in avoid-water stress test( p 0. 01 or p 0. 05). Conclusion: CHSGD treats IBS through downregulating expressions of serum IL-1β and IL-6,partly normalizing irregular HPA axis,lowering the content of colon 5-HT,CRH-R1 mRNA and intestinal hypersensitivity.
